Items required: 11 x Green Vest-First Aider (TCTA logo), 4 x Safety Bib-Fire Marshall (TCTA logo), 10 x SHE Rep Safety Vest (TCTA logo), 1 x Evacuation Coordinator Vest (TCTA logo), 6 x Evacuation Marshall Safety Vest (TCTA logo), 5 x Regulation First Aid Kit in Grab Bag (Red, TCTA logo), 1 x Chain saw trouser (SABS approved, XXL), 4 x Chain saw trouser (SABS approved, XL), 5 x Chainsaw helmet/face protection (SABS approved), 5 x Chainsaw protective bodyguards/pads (SABS approved), 5 x Chainsaw gloves (SABS approved). Company experience: Must have completed at least two (2) projects for supply/delivery of goods. Deliverables: As per scope of work.
Pricing must be submitted in the official Pricing Schedule (SBD6.1) and include: Cost per unit (excluding VAT), total cost (excluding VAT), and VAT @15%. Prices must be firm, unconditional, and include all disbursements. Conditional prices are not acceptable. Pricing schedule may not be modified or reproduced. Delivery cost must be included as a line item.
Tax Compliance: Submit TCS PIN (via SARS e-filing) or CSD Registration Report (if no TCS PIN). CSD number must be provided if applicable. B-BBEE: Submit valid B-BBEE Certificate (SANAS-accredited) or Sworn Affidavit (EME/QSE). Joint ventures must submit consolidated B-BBEE or individual affidavits. Sworn Affidavits must comply with Annexure B requirements (e.g., deponent details, black ownership %, revenue, VAT number, Commissioner of Oath stamp). Non-compliant or withdrawn agency certificates score 0 points. Mandatory forms: SBD1 (Invitation to Bid), SBD4 (Bidder’s Disclosure), SBD6.1 (Preference Points Claim). Foreign suppliers: Additional compliance requirements apply (see SBD1 Part A).
