Bids must be sealed, use official forms (not re-typed), and include all mandatory returnable documents. Late or incomplete bids will be disqualified. Stage 1A (Mandatory Requirements): Failure to submit any of the following results in automatic disqualification: (a) Completed Price Schedule and Pricing/Delivery Form (Section 4), (b) Signed Submission Register at SCM Offices, (c) Valid SANAS ISO/IEC 17025 Accreditation Certificate (must belong to the bidding service provider, not a third party). Stage 1B (Other Mandatory Requirements): Must be submitted within 7 days of award request if not included in bid: (a) Company Registration documents, (b) Copies of Directors’ ID documents, (c) Valid Tax Clearance Certificate (valid on closing date) and SARS-issued PIN, (d) CSD supplier registration number, (e) Proof of Bank Account, (f) Completed and signed ALL RFQ documentation (declarations, SBDs). Stage 1C (Non-Mandatory for Scoring): Missing documents result in zero points for applicable criteria: (a) Audited Annual Financial/B-BBEE Certificate/Affidavit, (b) CIPC Documents/B-BBEE Certificate/Affidavit. Stage 2 (Scoring): Price (80 points) and Specific Goals (20 points). Price scoring uses the 80/20 Preference Point System formula: PS = 80 × (1 − (Pt−Pmin)/Pmin). Specific Goals: 10 points for EME 51% Black Owned (Audited Annual Financial/B-BBEE Certificate/Affidavit), 10 points for 51% Black Owned (CIPC/B-BBEE Certificate/Affidavit).
