• Failure to supply any required administrative documents may lead to your quote not being considered.

• You must complete and submit the Bidder’s Disclosure (SBD 4) form.
• You must submit a Central Supplier Database (CSD) Report.
• You must submit a detailed pricing structure.
• You must submit a valid B-BBEE Certificate or Sworn Affidavit.
• You must submit the completed Request to Quote form.
• Failure to supply any of these documents may lead to your quote not being considered.

Suppliers must be registered on the Central Supplier Database (CSD) with compliant tax status. Bidders must not be listed on the Register for Tender Defaulters or List of Restricted Suppliers. Bidders must disclose any state employment or relationships with procuring institution officials. Failure to provide required documentation will result in disqualification.

Supply the following safety equipment for a mobile planetarium:

• Crowd control barrier (Quantity: 5):

• Extendable, portable, rapid setup.
• Can be attached together.
• Colours: white and red or yellow and red.
• Lightweight, durable, compact when closed.
• Height: 1m. Extendable length: 1m to 5m per barrier.

• Steel flatbed/platform trolley with handle (Quantity: 1):

• Load capacity: 250kg.
• Dimensions (max): 900mm H x 920mm L x 620mm W.
• Folding handle optional for storage.
• Durable steel with 360° rotating wheels.

• Inflatable air blower with deflator (Quantity: 1 set):

• Air blower specs: Power 950W–1100W, Voltage 220V-240V, Air flow 590-600M3/Hr.
• Must have a fixed ring to hold deflator attachment. Cable length: 5m.
• Deflator must be compatible with the blower and made of durable material.

• Provide a detailed pricing schedule with a full cost breakdown, inclusive of 15% VAT.
• The schedule must include all applicable costs: item prices, delivery fee, any disbursements or escalations.
• Submit a formal, detailed quotation. No price variations are allowed after the award.
• Use the provided cost schedule summary table to list: Crowd control barrier, Steel trolley, Inflatable air blower with deflator.

• You must be registered on the Central Supplier Database (CSD) and provide your CSD report with supplier number and company details.
• Provide your Tax Status Verification PIN and tax registration number.
• Submit a valid B-BBEE Certificate or Sworn Affidavit to determine your B-BBEE status level.
• Complete and submit the Bidder’s Disclosure (SBD 4) form, declaring no state employment conflicts, no relationships with procuring officials, and no collusive practices.
• Declare that your company and its principals are not listed on the Register for Tender Defaulters or the List of Restricted Suppliers.
• Failure to have a compliant CSD registration or tax status will lead to disqualification.

• Points for specific goals are awarded for B-BBEE status based on black ownership:

• 50% or more black ownership = 20 points.
• Less than 50% black ownership = 10 points.
• 0% black ownership = 0 points.

• Proof required: B-BBEE certificate and CSD report.

• The proposal and appended documentation form the basis for a formal contract to be negotiated.
• A mere offer and acceptance does not constitute a contract.
• Iziko reserves the right to remove line items and change quantities based on budget requirements.
• The lowest or only quote may not be accepted. Iziko reserves the right to accept or reject any quote and withdraw the RFQ at any time.
• Any contingency budget is for Iziko's use only and requires prior email approval from the project manager to invoice separately.

• Quotes must be valid for 60 days from the closing date.
• The formal contract will be negotiated based on the proposal.
• Iziko reserves the right to alter line items and quantities.
• The lowest quote is not guaranteed acceptance.
• Bidders consent to the use of their personal information as per POPI Act requirements.

• Your bid will be disqualified if:

• The document is not filled in correctly or completely.
• It does not comply with the specifications.
• It is submitted after the closing date and time.
• Your company is not registered on the CSD or has a non-compliant tax status.

• Iziko may also consider the following in the award decision:

• Risk of fruitless and wasteful expenditure.
• Risk of an abnormally low bid.
• Risk of a material irregularity.
• If you are currently in litigation with Iziko.
• Your poor past performance record on previous Iziko projects.
