Evaluation is a 4-phase process: Phase 1: Mandatory Requirements - Must have accreditation with relevant bodies (e.g., HPCSA, SACSSP) and a Durban-based Health and Wellness Centre compliant with health and safety regulations. Failure to meet these disqualifies the bid. Phase 2: SCM Administrative Requirements - Must submit CSD/SARS PIN proof, SBD 4, SBD1, POPIA consent form, and SBD 6.1. Missing any of these may disqualify the bid. Phase 3: Functionality Criteria - Only bids passing Phases 1 and 2 are evaluated. Minimum 70% score required to proceed to Phase 4. Criteria include: Conceptual grasp (0-30 points), Key Personnel (0-30 points), Reference letters (0-20 points), Workplan (0-20 points). Phase 4: Price and Specific Goals - 80/20 system. Price weighs 80 points (must include VAT, quote all items). Specific Goals weigh 20 points: 51%+ Black Ownership = 20 points, else 0. Proof: CSD Report/CIPC/B-BBEE Certificate/Affidavit.

Scope: Provide comprehensive employee wellness services for 60-70 TIKZN staff over 3 years. Services include: 1) Physical, mental, psychological, and physio-social wellness support. 2) Financial and socio-economic wellness support. 3) Minimum 8 counselling/educational awareness sessions per year (online and on-site), including speaker coordination. 4) Employee Medical Surveillance: Pre-employment, yearly, post-employment, and other legally required assessments (5-day turnaround for results). 5) Injury on Duty (IOD)/Disease Management: Assessments, legally required reports (3-day turnaround), employee readiness support. 6) Occupational Health Doctor services for exams. 7) 24/7 Professional Support Line (Call Centre) for employees and immediate family. 8) Ergonomic assessments and implementation roadmap. 9) Management of behavioural issues (substance abuse, absenteeism, addiction). 10) Annual on-site wellness day event. 11) Monthly health/safety/wellness brochures. 12) Monthly, quarterly, and annual usage analysis reports.

Pricing must include all applicable taxes (VAT @15%). Quote all required items/quantities; partial quotes will be disqualified. Required pricing table: Employee Medical Surveillance (pre/post-employment), Occupational Health Doctor services, Counselling sessions (online/physical), Professional Support Line (online/face-to-face), IOD/Disease Management, Monthly service fee (based on 60 employees).
