• Supply and deliver the listed laboratory consumables and reference materials to Cydna Laboratories, 75 4th Street, Houghton 2198.
• Items include Gilson tips (1 ml, 5 ml, 10 ml, 10‑100 µL, 200 µL), sterile urine containers (120‑150 ml, individually wrapped, with COA and radiation‑sterilisation certificate, 50 000 units), sterile Petri‑dishes (90 mm – 30 000 units; 55‑60 mm – 3 000 units) with COA, microbiological reference strains (Enterococcus faecalis ATCC 29212, Somatic coliphage ΦX174, Legionella pneumophila ATCC 33152) and various formalin‑fixed fecal suspensions (each 5 units) – all must be supplied with a valid Certificate of Analysis.
• All items will be evaluated against the specification individually.

• Quote unit prices as indicated; prices must be exclusive of VAT unless otherwise noted.
• Total quotation value should include VAT where applicable.
• No specific bond, guarantee or payment‑term details are provided in the document.

• Supplier must be registered on the Central Supplier Database (CSD) and provide proof of CSD registration.
• Submit original, valid Tax Clearance Certificate (SARS PIN).

– MBD 9: Certificate of Independent Bid Determination.
