• Bids must be delivered to the bid box at the Department of Health, 18 College Street, Polokwane, Limpopo. The box is open 24/7.
• Submit before 11:00 on 26 May 2026; late bids will be rejected.
• Use the official SBD forms supplied (SBD 1 Invitation, SBD 3.1 Firm Price, SBD 4 Disclosure) – do not re‑type the forms.
• All required documents must be fully completed, signed and attached (company registration, specialist registration, CVs, malpractice cover, tax compliance PIN or CSD number, full CSD registration report, consortium agreements if applicable, GCC and SCC declarations).
• Failure to attach any required document or to sign the forms will invalidate the bid.
• Disqualification may also occur for state employees or related parties, or for non‑attendance at the compulsory virtual briefing (08 May 2026, 10:00, link on e‑tenders.gov.za).

• Phase 1 – Mandatory Requirements: proof of company registration, specialist (HPCSA) registration, anaesthetist/medical officer registration, CV with ≥3 years post‑registration experience, active malpractice cover. All must be attached; non‑compliance invalidates the bid.
• Phase 2 – Administrative Compliance: completion of the standard bidding documents (SBD 1, SBD 3.1, SBD 4, CSD report, consortium agreements if any) and signed GCC/SCC declarations. Responses marked ‘Comply’, ‘Not Comply’ or ‘Not Applicable’ as instructed.

Scope: Accreditation of a panel of ophthalmology specialists to provide ophthalmology services (cataract, glaucoma, squint, retinal, etc.) in designated Limpopo hospitals for 36 months, on an as‑and‑when‑required basis.

• Provider must supply own qualified ophthalmologist(s) and an anaesthetist or medical officer with a Diploma in Anaesthesia.
• Department supplies operating theatre, nursing staff and surgical supplies; provider supplies specialist fees and patient‑specific costs.
• Services include pre‑assessment, surgery, post‑operative review up to 4 weeks, and record‑keeping in patient files.
• Bidders must indicate capacity (number of operations per day) and submit per‑patient costing (prices inclusive of overheads, VAT and all taxes).
• Invoices must be accompanied by signed patient lists and clinical documentation.

• Pricing must be submitted per patient operated, inclusive of business overheads, VAT and all taxes.
• Price schedule valid for 12 months; CPI adjustment may apply annually after the first year.
• Bidders must be VAT‑registered at award; if not, registration must occur within 30 days.
