Bids must be in sealed envelopes marked 'PROVISION OF EMPLOYEE WELLNESS SERVICES' and include one (1) original hard copy and a soft copy (USB). All bid documents must be completed in non-erasable ink, remain intact, and use official forms provided. Required returnable documents: SBD 1 (Invitation to Bid), SBD 3.3 (Pricing Schedule), SBD 4 (Bidder’s Disclosure), SBD 6.1 (Preference Points Claim Form), SBD 7.2 (Contract Form – Rendering of Services), Resolution to Sign, Tax Compliance Status Pin, B-BBEE Certificate/Sworn Affidavit, Company Registration Documents, CSD Report, Signed GCC, Protection of Personal Information Consent Form, Consent for Credit and World Checks (Forms 1-4). Failure to submit any mandatory document may render the bid invalid. Bids must remain valid for 120 days from closing date. B-BBEE minimum level: 7. Key deliverables: 1) Strategy Development: Conduct diagnostic assessment (gap analysis, needs assessment, risk profiling), develop multi-year wellness strategy with pillars (mental health, physical wellbeing, financial wellness, organisational health), and create implementation roadmap with short/medium/long-term initiatives. 2) Executive Wellness Programme: Personalised, confidential, and preventative, including health risk assessments, individualised wellness plans (fitness, ergonomics, nutrition, stress management), premium facilities, and ongoing coaching. 3) Employee Wellness Programme: Holistic, inclusive, and preventative, covering psychosocial support, financial wellness, legal advisory, health promotion, and lifestyle management.
