Evaluation will be based on the following:

• Pre-qualification: Mandatory OEM certification (Komatsu or equivalent) for loaders and haul trucks, and documented proof of experience in repairing loader buckets, ground engagement tools (GETs), and equivalent equipment (e.g., Komatsu 730E, WA1200, PC5500).
• Technical/Functional Evaluation: Documentary proof of capacity to meet scope requirements (e.g., on-site labour, workshop, tools, and equipment).
• Price and B-BBEE: Price offer, price schedules, and B-BBEE certificate (minimum Level 1 required).
• Failure to submit requested documentation will result in disqualification.

Scope: Long-term repair and maintenance (RAMP) for Komatsu loaders (WA1200, PC5500) and haul trucks (18 units) at Foskor Phalaborwa mining division.

Key Requirements:

• Maintenance Types: Full Life Cycle Cost (LCC) based RAMP for WA1200 loaders; partial LCC for PC5500 shovels. Includes major bucket, lubes, and component repairs/replacements.
• Equipment Coverage:
• Komatsu PC5500 Face Shovels (2 units, ages: 106,166 and 76,166 hours).
• Komatsu WA1200 Front Loaders (2 units, ages: 29,847 and 9,508 hours).
• 18 Komatsu haul trucks (various models).
• Ground Engagement Tools (GETs): Annual provision for 2 interventions/year per equipment (WA1200, PC5500).
• Bucket Repairs: Weekly inspections, six-monthly interventions, and yearly rebuilds (2 interventions/year per equipment).
• On-Site Labour: Minimum complement required:
• 1 Foreman, 4 Technicians, 4 Assistants, 8 Millwrights (haul truck support), 1 Cummins Man-On-Site support.
• Service Vehicles: 4 LDVs, 1 Maintenance Service Truck.
• Oils and Lubricants: Supplied per grease and lubricant schedules (tables provided in document).
• Work Standards: Must meet Komatsu OEM standards, with workmanship guarantees for repairs.
• Work Hours:
• Office: Mon-Thu 06:30-15:45, Fri 06:30-12:45.
• Call-outs: Response within 45 minutes after hours; 24/7 standby required.
• Reporting: Monthly reports summarizing labour hours (normal/overtime) and costs per category.
• Safety: HIRA and Safe Work Procedure required before work commencement; compliance with Mine Health and Safety Act (MHSA) and Foskor COPs (e.g., COP 25, COP 59).
• Quality Assurance: ISO 9001:2015 alignment; regular SHEQ inspections by Foskor.
• Penalties: Rework within OEM/Komatsu guarantee period covered under warranty.

Exclusions/Inclusions: Foskor provides site workshop space, cranage, office space, ablutions, electricity, and potable water. Contractor supplies labour, equipment, tools, and workshop setup.

Pricing Structure:

• Submit a 3-year pricing schedule (excl. VAT) for:
• Maintenance costs (lubes, standard spares, fixed/operating costs) for each equipment type (PC5500, WA1200, haul trucks).
• Provision for GETs and bucket repairs (minor and refurbishment) as per BOQ Attachments 1 and 2.
• Minimum service parts for equipment under full maintenance (estimate costs per unit provided in document).
• Pricing Format: Breakdown per year (Year 1: Dec 2025–2026, Year 2: 2026–2027, Year 3: 2027–2028) with total spend over 3 years.
• Escalation: Include annual escalation rate (subject to negotiation).
• VAT: 15% to be added to grand total.
• Payment Terms: Monthly invoices must be submitted on time for approval and processing.

Mandatory Compliance:

• OEM Certification: Komatsu or equivalent OEM certification for loaders and haul trucks (submit OEM letter or proof of approval).
• B-BBEE: Minimum Level 1 certificate required (submit with bid).
• CSD Registration: Proof of CSD registration or MAAA number.
• CIPC/Company Registration: Submit registration documents.
• Tax Compliance: SARS PIN and tax clearance certificate.
• COIDA: Letter of Good Standing.
• Financial Statements: Submit latest financial statements.
• Beneficial Ownership: Certificate required.
• Legislative Compliance:
• Mine Health and Safety Act (MHSA) with Regulations.
• National Road Traffic Act with Regulations.
• Foskor COP 25 (Service Provider Control) and COP 59 (Trackless Mobile Machinery).
• All Foskor SHEQ, quality, and environmental procedures.
• Safety Requirements:
• Appoint 2.6.1 (subordinate manager), 2.9.2 (on-site supervisor), and SHE Rep (Section 29(1) of MHSA) within 30 days of award.
• Compile a Safety File (per Foskor standards) including:
• Permit to Work (obtained from Foskor Safety Department).
• Baseline Risk Assessment (BRA) for all tasks.
• Training records (e.g., HIRA, First Aid Level 1, Working at Heights, Basic Fire Fighting, Lock Out).
• PPE and equipment inspection records.
• Medical examinations (entry/exit) for all on-site employees at Foskor Clinix Clinic (R800/person, valid for 1 year).
• Vehicle Compliance:
• Drivers must have valid national licenses, Foskor competence testing, and MHSA 2.13.1 authorization.
• Service vehicles require Foskor COF (Certificate of Fitness) and access disk (contact Light Vehicle Maintenance Workshop: 015 789 2840).
• Daily pre-use inspections per COP 59.
• Permit to Work: Allow 3–10 working days to complete all requirements (contact Foskor Safety Department: 015 789 2547).
• Local Content: Encouraged to use local sub-contractors for non-core services to meet mining industry legislative requirements (local procurement, supplier development, CSI).
