Compulsory returnable documents: 1. National Treasury Central Supplier Database (CSD) number; 2. Company name; 3. Valid SARS Tax Compliance Pin (with supporting letter); 4. Duly completed and signed SBD 4 (Bidder’s Declaration); 5. Duly completed and signed SBD 6.1 (Preference Points Claim Form). Non-submission of any required document will disqualify the bid.

Scope: Dismantling, re-assembling, and wrapping of all office furniture and related equipment; packaging of approximately 300 boxes of current files; transportation, loading, and off-loading; placement of furniture at the new office; and insurance. All services must be performed from 20 Paul Kruger Street, ABSA Square Building, Mbombela, to a destination within a 5-kilometer radius. Pricing must include VAT at 15%.

Provide a detailed pricing breakdown for: 1. Dismantling, re-assembling, and wrapping of furniture; 2. Loading and off-loading; 3. Transportation; 4. Placement of furniture at the new office; 5. Insurance. Include Sub-Total, VAT (15%), and Grand Total.

Bidders listed in the Register for Tender Defaulters or List of Restricted Suppliers will be disqualified.

Preference points for specific goals: Women ownership (100%) = 6 points; Youth ownership (100%) = 6 points; People with Disabilities ownership (100%) = 4 points; Black ownership (100%) = 4 points. Submit supporting documents: CIPC registration, certified IDs (Women/Youth/Black), medical letter (Disability). Non-submission results in zero points for specific goals.

Bidders must certify independence in bid preparation and non-disclosure of bid terms to competitors. Collusion, fraud, or non-compliance may lead to disqualification, cost recovery, contract cancellation, public sector restrictions, or criminal prosecution. The organ of state reserves the right to verify claims and report suspicious bids to the Competition Commission or NPA.

Bidders must disclose: 1. State employment of directors/trustees/shareholders; 2. Relationships with procuring institution employees; 3. Interests in related enterprises. Failure to disclose may result in disqualification or other penalties.

For disposal or leasing of state assets, price scoring uses: Ps = 80(1 + (Pt - Pmax)/Pmax) or Ps = 90(1 + (Pt - Pmax)/Pmax), where Pmax = highest acceptable tender price. This does not apply to this RFQ.
