Bids must be submitted on the official bid document provided by Mpofana Local Municipality, deposited in the official tender box at 10 Claughton Terrace, Mooi River 3300, before the closing date and time. Emailed, faxed, or late bids will not be accepted. Bids must be clearly marked with the bid number and description. Use of Tipp-Ex or completion in pencil invalidates the bid; no pages may be removed; all forms must be completed without amendment. The 80/20 preference point system applies. Price is allocated a maximum of 80 points, calculated using the formula Ps = 80(1 - (Pt - Pmin)/Pmin), where Pt is the tender price and Pmin is the lowest acceptable tender price. Specific goals are allocated 20 points: 100% Black Owned (10 points), 100% Black Woman Owned (5 points), Disability (5 points). Bidders must indicate in writing how they claim points for each specific goal and submit supporting documentation; failure to do so means no preference points are claimed. Scope: Appointment of a panel of healthcare practitioners to provide occupational health and employee wellness services on an as-and-when-required basis for 36 months. Services include: occupational medical surveillance (occupational history, physical examination, blood pressure, height/weight, BMI, urine analysis, vision and colour vision testing, audiometry, spirometry, biological monitoring, drug and alcohol screening, chest X-rays, lung function, fitness for work); periodic medical examinations; pre-employment medical examinations; medical evaluation referrals (fitness for duty, ill-health retirement, medical boarding, second opinions); pension fund medical assessments; psychological assessments (trauma counselling, clinical consultations, wellness interventions, therapeutic services) by an HPCSA-registered psychologist; and specialised services (chest X-rays, specialist referrals, pathology, medical opinions). Service delivery may be at the provider's registered medical facility or via a fully equipped mobile clinic. Facilities must be a fully operational medical practice registered in South Africa, located within the uMgungundlovu District, or a compliant mobile occupational health clinic. Where X-ray services are not on site, bidders must indicate arrangements with an accredited radiology practice. Reporting: after each assessment, submit Employee Fitness Certificate, Individual Medical Report, Medical Boarding Report (where applicable), Pension Fund Medical Report (where applicable), Psychological Assessment Report (where applicable), Annual Occupational Health Report, and signed Attendance Register. Reports must be submitted within 5 working days of assessment. Service levels: schedule appointments within 5 working days of referral, provide emergency appointments where required, maintain accurate medical records, and keep all employee information confidential. Contract handover: cooperate with the municipality and any new provider upon expiry or termination, subject to confidentiality and data protection requirements.

Bidders must have specific experience and submit at least two recent references (in the form of written proof(s) on municipality's letterhead including relevant contact person, nature of service, contract amount, commencement date, telephone number, and email addresses) of similar work undertaken.

Medical surveillance must include occupational history assessment, physical examination, blood pressure, height and weight, BMI, urine analysis, vision and colour vision testing, audiometry, spirometry, biological monitoring, drug and alcohol screening (where required), chest X-rays (where applicable), lung function assessments, and fitness for work evaluations. Periodic medical examinations must be conducted in accordance with applicable legislation and occupational risks. All reports must be submitted within five working days of assessment.

Failure to fulfil these obligations constitutes a breach of contract.

Bidders must be registered on the Central Supplier Database (CSD) and provide a CSD summary report. Tax compliance: submit a valid Tax Clearance Certificate or SARS PIN; a printed TCS certificate may be submitted. For consortia/joint ventures/sub-contractors, each party must submit a separate TCS certificate/PIN/CSD number. Bidders must provide CIPC registration. Professional registration: healthcare practitioners must be registered with HPCSA, SANC, SASOM, or any other relevant statutory council; proof of current registration must accompany the bid. B-BBEE: submit a B-BBEE status level verification certificate or sworn affidavit (for EMEs and QSEs) to qualify for preference points. Bidders must complete MBD 4 (Declaration of Interest) disclosing any state employment or relationships with state officials; MBD 8 (Declaration of Past SCM Practices) confirming no abuse of SCM, no fraud/corruption convictions, no tender default listing, no municipal arrears over three months, and no terminated contracts due to non-performance; and MBD 9 (Certificate of Independent Bid Determination) certifying no collusive bidding. Bidders must comply with the Occupational Health and Safety Act, COIDA, Labour Relations Act, Basic Conditions of Employment Act, Employment Equity Act, POPIA, National Health Act, and HPCSA Ethical Rules. Minimum experience: at least 3 years' relevant experience in occupational health, experience conducting medical surveillance, knowledge of municipal working environments, and experience with COIDA and OHSA.

Providers must have at least three years' experience in occupational health and knowledge of municipal working environments. The contract will include terms and conditions as per the General Conditions of Contract (GCC) and any special conditions.
