Submitting more than one email will result in disqualification. Ensure all forms are completed, signed, and dated; do not retype or redraft forms.

The tender follows a three-phase evaluation: 1) Administrative Compliance (checking CSD registration, SBD forms, tax compliance, and signatures), 2) Mandatory Requirements (verification of medical practitioner qualifications and HPCSA registration), and 3) Price and Preference (80/20 system where 80 points are for price and 20 for specific goals).

The service provider must certify pre-employment and exit medical assessments for road construction personnel (15 incumbents each). The medical practitioner must: 1) Hold an MBChB degree, 2) Possess a postgraduate degree in Occupational Medicine or Occupational Health, 3) Have at least three years of professional experience, and 4) Be registered with the HPCSA. All qualifications and registration must be for the same individual.

Pricing must be submitted using the provided SBD 3.3 form. All delivery costs must be included in the bid price. The quote must be valid for 90 calendar days from the closing date.

Must be registered on the Central Supplier Database (CSD). Must provide a valid Tax Compliance Status (TCS) PIN or CSD number. Must complete all SBD forms (SBD 1, 3.3, 4, 6.1). Must provide proof of HPCSA registration and relevant medical qualifications for the practitioner. B-BBEE status level verification certificate or sworn affidavit required for preference points.
