3. Fully completed SBD 4 declaration form (date must match quotation date). Using a different form will cause disqualification.

4. Proof of Central Supplier Database (CSD) registration.

5. Originally certified copies of directors' ID documents (not older than 6 months).

6. Company profile.

7. Municipal rates and taxes statement in the company's name.

8. Original or certified copy of a valid BBBEE Certificate (SANAS accredited) or a BBBEE sworn affidavit signed by a Commissioner of Oaths.

• For business address verification:
• If operating from a director's residence: attach municipal rates and taxes in the director's name plus an affidavit confirming business operation at that address.
• If operating from leased premises: attach a valid lease agreement in the company's name.
• If using a council letter: it must be in the director's name, accompanied by an affidavit; council letters in the company's name are not accepted.
• Quotations must remain valid for 60 days.
• Persons in the service of the state are not allowed to quote.
