• Ensure your quotation includes all required supporting documents; failure to submit any will result in immediate disqualification:

1. Company registration documents (CIPC).
2. Valid Tax Clearance Certificate.
3. Fully completed SBD 4 Declaration form (date must match quotation date).
4. Proof of Central Supplier Database (CSD) registration.
5. Originally certified copies of directors' ID documents (not older than 6 months).
6. Company profile.
7. Municipal rates and taxes statement in the company's name (or director's name if operating from residence, with affidavit).
8. Valid BBBEE Certificate (SANAS accredited) or sworn affidavit.

• For leased premises, attach a valid lease agreement in the company's name.
• Quotations must remain valid for 60 days.
• Persons in the service of the state are not allowed to quote.

• Failure to submit required documents results in immediate disqualification. Required documents include company registration (CIPC).

Service providers must be registered on the Central Supplier Database (CSD), not be in the service of the state, and provide all required supporting documents. Failure to submit any required document results in immediate disqualification. Business address verification is mandatory (via municipal statement, lease agreement, or council letter with affidavit).

• Deep cleaning of ablutions (quantity: 1).

• Provide company profile.
• Submit certified copies of directors' ID documents.
• Provide proof of business address via municipal statement or lease agreement with affidavit.

• Mandatory registrations and documents:

• Registration on the Central Supplier Database (CSD). Proof must be attached.
• Company registration with CIPC.
• Valid Tax Clearance Certificate.
• BBBEE compliance: Submit a valid BBBEE Certificate (SANAS accredited) or a sworn affidavit.

• Additional proof of business address required:

• Municipal rates and taxes statement, or lease agreement with affidavit as applicable.
