• Quotation must be valid for 60 days from the submission date.
• Prices must be quoted per the unit indicated, exclusive of VAT (VAT to be added where applicable) and must include the supplier’s authorised signature.
• Late submissions (after 16:00 on 11 Feb 2026) will be rejected.
• Failure to provide brand names where required, or to include the authorised signature, will result in disqualification.

• Scoring follows an 80/20 preference point system:
• 80 points allocated to price (lowest acceptable price receives full points; points calculated using the formula provided).
• 20 points allocated to specific goals as per the Preferential Procurement Regulations 2022.
• Specific goals include:
• SMME status (EME or QSE) – up to 20 points (51% ownership or more black‑owned).
• Valid BBBEE certificate or sworn affidavit.
• SANAS accreditation verification.
• Preference points must be claimed with supporting documentation; failure to provide proof means the points are not awarded.
• The organ of state may request additional evidence at any stage.
• Total possible score is 100 points (price + specific goals).

• Provide pathology and laboratory services for a Johannesburg Water clinic, covering routine and specialized tests.
• Supplier must be accredited by SANAS and comply with all relevant legislation.
• Proof of SANAS accreditation must be submitted with the quotation.
• Example tests that may be required (list not exhaustive):

1. Hepatitis A (IgG) – 200 units

2. HBA1c – 30 units

3. Viral Load – 15 units

4. CD4 count – 15 units

• Service provider must be able to deliver the above tests and any other tests as may be specified during the contract period.

• Prices must be quoted per the unit indicated in the quotation schedule, exclusive of VAT (VAT to be added where applicable).
• Total quotation value must include VAT where applicable.
• Quotation must remain valid for 60 days.
• No specific bond, guarantee or payment term is stipulated in the document.
• Submissions must be signed by an authorised representative of the supplier.

• Supplier must be registered on the Central Supplier Database (CSD) – supplier number required.
• Provide a valid Tax Clearance Certificate (SARS PIN).
• Provide a valid BBBEE certificate or a sworn affidavit compliant with the B‑BBBEE Practice Guide.
• Submit a copy of a valid lease agreement or municipal account (not older than 3 months).
• Submit completed MBD forms: Declaration of Interest (MBD 4), Supply Chain Management practices (MBD 8), Independent Bid Determination (MBD 9), Preference points claim (MBD 6.1).
• Provide company registration documents with ID copies of directors/shareholders.
• Provide proof of SANAS accreditation.
• Provide proof that municipal rates and taxes are not in arrears for more than 90 days.
• All the above documents are compulsory; failure to submit any will result in disqualification.
