Scope: Conduct Occupational Hygiene Surveys at various Johannesburg Water sites (Ffennell Road Depot, Cydna Laboratories, Northern Works, Randburg Depot, Head Office, Midrand Depot, Helderkruin Depot, Zandfontein Region, Goudkoppies Lab). Surveys must assess: Noise (70 samples), Ergonomics (70), Illumination (100), Indoor Air Quality/Ventilation (100), Hazardous Biological Agents (60), Thermal Stresses (100), Asbestos (3), Hazardous Chemical Agents (30). Additional requirement: Present results to management and employees on-site once reports are finalized (1 session). Service provider must provide: SANAS 17020:2012 accreditation, Department of Labour registration as an Approved Inspection Authority (AIA), Occupational Hygienist certificates (with related qualifications and SAIOH registration), CVs of key personnel, and two signed reference letters from previous/current clients (including client name, service description, contact details). Reports must include interpretation of results and recommended control measures to protect employees.

Pricing must be firm (no price adjustments for exchange rates or other variables). Total quotation value must be inclusive of all applicable taxes (VAT, PAYE, income tax, UIF, SDL). Non-compliance with tax inclusivity will lead to disqualification. Submit pricing via MBD 3.1 (Pricing Schedule). Offer validity period: 60 days from closing date. Delivery costs must be included in the bid price. Separate pricing schedules required if different delivery points influence pricing. All quotes must be on company letterhead and in PDF format.
