The candidate information verification, consent and declaration addendum must be completed by each candidate and submitted through the appointed recruitment service provider as part of the recruitment submission process. The addendum requires the candidate's full details (full names and surname, identity/passport number, nationality/citizenship, residential address, email address, telephone number, position applied for, recruitment service provider name, date of submission) and the candidate's signature and date. The recruitment service provider must also confirm IT has interviewed/screened the candidate, discussed the relevant position, obtained the candidate's authority to submit, and taken reasonable steps to verify material information, with the provider's representative name, signature and date.
