The SBD4 Bidder's Disclosure form imposes the following compliance requirements:

• Declaration of state employment for the bidder and all persons with a controlling interest (Section 2.1). If yes, full names, identity numbers, and state institution names must be provided.
• Disclosure of any relationship with employees of the procuring institution (Section 2.2).
• Disclosure of any interest in other related enterprises, whether bidding or not (Section 2.3).
• Certification that the bid was prepared independently, without collusion, consultation, or agreement with competitors (Sections 3.3–3.4). Communication between joint venture/consortium partners is excluded.
• Certification that bid terms have not been disclosed to competitors before bid opening or contract award (Section 3.4).

• Acknowledgement that false disclosure may lead to bid rejection, reporting to the Competition Commission (section 59 of Competition Act 89 of 1998), criminal investigation by the NPA, and restriction from public sector business for up to 10 years under the Prevention and Combating of Corrupt Activities Act 12 of 2004 and PFMA SCM Instruction 03 of 2021/22.
• Signature, date, position, and printed name of the authorised signatory.
