Required forms: SBD1 (Invitation to Quote), SBD3.1 (Pricing Schedule), SBD4 (Declaration of Interest), SBD6.1 (Preferential Points Claim). Additional requirements: Latest CSD report, bank confirmation letter (not older than 3 months), VAT registration number, and supplier compliance status (Tax Compliance Central Supplier Database). Failure to submit any required document may invalidate your quote.

Scope: Rate-based contract for plumbing repairs and maintenance at Frere Hospital, as and when required, over 12 months. Maximum contract value: R1 million. Services include: supply/install geysers, basins, mixers, toilets, scrub sinks, unblocking sewer pipes, manholes, urinals, hydro-boils, servicing water fountains/pools/pumps, and pipe/fittings (PVC/Galvanized Iron). Labour rates: Normal hours (07:00-17:00), after-hours (17:00-07:00), weekends (Friday 17:00-Monday 07:00). Unspecified items require proven cost + agreed mark-up. Work Orders (W/O) issued for repairs; invoice monthly per completed job cards. Contract may be cancelled if long-term or provincial/national contracts are awarded. No guarantee of work volume; subject to budget availability. Site must be kept clean and safe; remove all building rubble. Standards: Pre-cast concrete manholes (1m+), 110mm PVC underground sewer piping, Cobra heavy-duty taps (elbow-action in clinical areas), Springbok hand basins, Franke scrub sinks, Kwik Hot geysers.

Premises and work subject to inspection by Department or its representative at reasonable hours. Pre-bidding testing costs borne by bidder. If inspections reveal non-compliance: Costs borne by supplier; supplies/services may be rejected. Supplier must remove/replace non-compliant items at own cost. Purchaser may arrange inspections during contract period and cover costs if compliant. Supplier must permit record audits if required.

Submit firm prices only via SBD3.1 (Pricing Schedule). Include all applicable taxes (VAT, PAYE, UIF, SDL). Quotation validity: 120 days from closing date. Total contract cost cannot exceed R1 million. Unspecified parts: Claim proven cost + agreed mark-up (specify % in bid). Rates must cover labour, materials, and all obligations. Example items: Geyser installation (150L), basin replacement, mixer replacement, toilet replacement, sewer unblocking, manhole construction, hydro-boil installation, water fountain servicing, pool servicing, pump servicing/replacement, PVC/galvanized iron pipes and fittings, cherry picker hire, labour rates (normal/after-hours/weekend).

Pricing: Submit firm prices only (non-firm prices rejected). Use SBD3.1 for pricing schedule. Include all applicable taxes (VAT, PAYE, income tax, UIF, SDL). Total contract cost cannot exceed R1 million over 12 months. Payment: Within 30 days of invoice submission with delivery note. Quotation validity: 120 days from closing date. Unspecified parts: Claim proven cost + agreed mark-up (specify % in bid).

Mandatory: PIRB (Plumbing Industry Regulatory Board) registration to provide Certificate of Compliance (CoC). 3 contactable references for plumbing work completed in the last 24 months (attach completion certificates and reference letters). Tax Compliance: Submit SARS TCS PIN or CSD number. CSD registration required. B-BBEE: Preference points for Specific Goals (HDI, Women, Youth, Disability, Military Veterans, Locality Ownership). Submit proof (CIPC certificates, municipal accounts, medical/disability/veteran status documents). Foreign suppliers: If no RSA presence, no TCS PIN required. State employees/directors/members in service of the state are disqualified.

Contract governed by General Conditions of Contract (GCC) and Special Conditions of Contract (SCC). Key terms: Contract cannot exceed R1 million or 12 months. Work commences only after official Purchase Order from SCM. No work without PO is chargeable. Budget availability influences contract operation. Frere Hospital reserves right to award/partially award or not award. Contract may be cancelled if long-term or provincial/national contracts are established. Payment within 30 days of invoice + delivery note. Prices must remain firm. Contract may be terminated for default, insolvency, or force majeure. Disputes resolved via mediation then South African courts. Supplier liable for penalties for delays (prime interest rate per day).

Three-stage evaluation: 1) Administrative Compliance: SBD1, SBD3.1, SBD4, SBD6.1, latest CSD report, bank confirmation letter (≤3 months). 2) Non-Negotiable Requirements: PIRB registration + 3 contactable references (plumbing work in last 24 months). Failure in Stage 1 or 2 disqualifies bid. 3) Price (80 points) + Specific Goals (20 points). Price: Lowest acceptable tender scores highest (Ps = 80*(Pmin/Pt)). Specific Goals: HDI Ownership (4), Women Ownership (4), Youth Ownership (2), Disability Ownership (2), Military Veterans Ownership (2), Locality Ownership (6).
