Telegraphic, telefax, emailed or other electronic submissions are not accepted. Documents must be submitted intact (NO loose pages) and completed in black ink; use of correction fluid (tippex) is prohibited. All price alterations must be signed. The total price inclusive of VAT must be written in words and figures on the form of offer and signed; failure to do so means the offer will not be recorded in the tender opening register and will be evaluated as having NO offer. Returnable forms to be completed, signed and submitted: mbd 1 (invitation to bid), mbd 4 (declaration of interest), mbd 6.1 (Preference points claim), mbd 6.2 (Local production and content declaration), form of offer, and any other forms included in the bid pack. Company profile with proven previous experience must be attached. Certified copies of directors' ids (certified within 3 months), company registration document (ck), valid SARS tax compliance status pin or csd number, proof of municipal rates account not in arrears >30 days (or lease/affidavit/ward councillor letter), and comprehensive jv agreement (if applicable) must be included. Bids remain valid for 90 days after closing.
