Non-compliance will result in disqualification. • Mandatory returnable documents: - recent central supplier database (csd) report or csd unique number. - Completed, signed sbd 4 and sbd 6.1 Forms. - Original or certified b-bbee certificate or sworn affidavit (sanas logo must be visible). • Bid validity period: 90 days from the RFQ closing date. • Failure to meet any submission requirement (format, documents, deadline) will lead to disqualification.

- Must be a qualified service provider in health and wellness campaigns. - Campaign must be delivered at raf hatfield office. - Duration of the campaign is two (2) days. - Submission must align with the request for quotation (RFQ) process. - Target audience and activities should focus on promoting healthy lifestyles. - Compliance with raf’s procedural and ethical standards is required.

Namc will not be liable for costs incurred without a po. • Ensure all pages are numbered and NO duplicates/missing pages exist. Namc accepts NO liability for errors arising from missing/duplicated pages.
