• Ability to provide conflict of interest training sessions • experience in similar training services • registration with relevant authorities (e.g. CIDB, BBBEE)

• Service provider must be able to provide stat and routine clinical laboratory and anatomic pathology services • the services are required for sanpc refinery employees • the contract period is three years

• Proven epc and o&m capability for dms coal washing plants • CIDB grading 6me or higher (mining infrastructure) • valid BBBEE certificate and five-year financial projections

Return a scanned, signed copy of the completed cef group kyc questionnaire to cef group nominated personnel. Attach all required supporting documents as listed in the questionnaire. Additional pages may be used where necessary.

• Complete the sbd4 bidder’s disclosure form and attach IT to the bid submission. • The form must include declarations on state employment, relationships with procuring institution staff, and interests in related enterprises. • If the bidder or any controlling person is listed on the register for tender defaulters or the list of restricted suppliers, the bid will be automatically disqualified. • All declarations must be truthful; false statements will lead to immediate disqualification and possible legal action. • Submit the completed form together with the technical and financial bid documents by the closing date specified in the tender notice. • Ensure the signature, date and position of the authorised signatory are clearly indicated on the disclosure form.

Include the preference points claim form (annexure c) with your quotation. The form must be completed in full and signed, and any claimed specific goal points must be supported by the required documentation at the time of submission. Failure to provide proof will result in loss of those preference points.

• Bids may be emailed to [email protected]. • All bids must be delivered by the stipulated closing time (12h00 on 16 february 2026) to the correct address; late bids will be rejected. • Use the official forms provided in the bid documents – do not re‑type the forms. • The successful bidder must complete and sign the written contract form (sbd7). • Include required returnable documents (e.g., Proof of authority such as a company resolution). • Disqualification risks: late submission, failure to use official forms, missing tax compliance certificates or pin, or being a person in the service of the state.

• Complete the mandatory cef kyc questionnaire in full. • Use additional pages if needed. • Sign the questionnaire. • Attach all required supporting documents listed in the questionnaire. • Return a scanned copy to the cef group nominated personnel (address and email to be provided by cef).
