• Service provider responsibilities:
• Check, repair, or replace malfunctioning machines.
• Refill dispensers with water weekly or as required.
• Mandatory technical requirements:
• Provide latest water lab test results.
• Provide a valid SANBWA Certificate.
• Provide three reference letters for similar work/services in the past 5 years.

• Quote must include:
• Provision of water dispensers (quantity: 4).
• Call-out fee (08:00–17:00).
• Price per bottle refill.
• Travel rate per km.
• Mark-up percentage for consumables.
• Yearly totals for a 3-year period with escalation.
• All prices must be quoted in Rand, firm, and inclusive of VAT where applicable.
• Payment terms: 30 working days after receipt of a valid invoice.

• Mandatory documents to submit:
• Central Supplier Database (CSD) Report or Summary.
• Completed and signed SBD 4.
• Completed and signed SBD 6.1.
• Certified valid B-BBEE Certificate or sworn affidavit.
• Valid Tax Reference Number and PIN.
• VAT number if applicable.
• Tax compliance status will be verified via CSD or SARS e-Filing. Non-compliance allows 7 working days to rectify; failure leads to disqualification.
• Bidders must respond to clarification requests within stipulated times (e.g., 48 working hours for SBD 4, 7 working days for tax status) or be deemed non-responsive.
• Disqualification reasons include:
• Non-compliant tax status.
• Fraudulent or inaccurate information (e.g., B-BBEE credentials, experience).
• False declarations on SBDs.
• Listing on National Treasury’s Database of restricted suppliers.
• Failure to quote as per specification.
• The RFQ is subject to general conditions of contract (GCC) and any SANAS special conditions.
