Bids must be deposited in the bid box at nkangala district municipality, 2a walter sisulu street, middelburg, 1050, by 12:00 on 04 september 2026. Late bids will not be accepted. Bids must be submitted on the official forms provided (not re-typed) or online. The successful bidder will be required to sign a written contract form (mbd7). Returnable documents (all must be completed and signed; failure to fully complete any compulsory document renders the tender unresponsive): - form a: mbd 1 (invitation to bid, parts a & b) — bidder details and acceptance of terms. - Form b: compulsory enterprise questionnaire for consortia or joint ventures — special resolution, joint and several liability, 30 days' notice to terminate. - Form c: popi notice and consent form — consent to the municipality processing and sharing personal information for verification purposes. - Form d: declaration of bidder's past supply chain management practices (mbd8) — discloses any abuse of SCM systems, fraud/corruption convictions, contract failures, or listing on restricted supplier registers. - Form e: declaration of interest (mbd4) — discloses any connection to persons in the service of the state; full details of directors/trustees/shareholders mandatory. - Form f: authority of signatory — proof of authority to sign on behalf of the company, on company letterhead. - Form g: declaration of good standing regarding tax (mbd2) — tax compliance status. - Form h: financial references — bank letter not older than 3 months confirming account details. - Form i: municipal utility account — declaration that NO municipal rates/taxes are in arrears, with utility account or lease agreement attached. - Form j: preference schedule (mbd 6.1) — Claim for preference points. - Form k: certificate of independent bid determination — certifies NO collusion. - Form l: declaration of tenderer's litigation history. - Form m: proof of registration with professional bodies. - Form n: professional indemnity insurance. - Forms o–u: evaluation returnables — project structure, key personnel, previous/current experience, infrastructure/resources, sub-contractors, addenda record. Disqualification risks: - any compulsory returnable document omitted or incomplete. - Late submission. - False declarations — treated as wilful misrepresentation and may lead to contract termination under contract condition 8.4. - Bids from persons in the service of the state will not be considered.
