• Submit all required sbd forms as listed: sbd 1 (invitation to bid), sbd 2 (tax clearance certificate), sbd 3.1 (Pricing schedule), sbd 4 (declaration of interest), sbd 5 (national industrial participation programme), sbd 6.1 (Preference points claim), sbd 7.1 (Contract form), sbd 8 (supply chain management practices), sbd 9 (certificate of independent bid determination) and fronting declaration. • Include cvs of the two key project support personnel (minimum experience required – see evaluation criteria). • Provide sample individual and organisational reports as per the technical specifications. • Attach at least five verifiable reference letters (client name, contact, completion date, service description). • All documents must be signed, dated and submitted together with the bid by the closing date. • Failure to submit any of the above returnable documents may lead to disqualification.
