Returnable documents — all must be completed, signed and submitted with the bid: - mbd 1 (invitation to bid): bidder and contact details, signed as the offer cover page. - Mbd 4 (declaration of interest): discloses any director or shareholder employed by the state or connected to anyone at the municipality. - Mbd 5 (declaration for procurement above r10 million): required if the contract value exceeds r10 million; includes audited financial statements and disclosure of outstanding municipal debts. - Mbd 6.1 (Preference points claim): claims specific goal points under the 80/20 system. - Mbd 8 (declaration of bidder's past SCM practices): certifies NO abuse of supply chain systems, NO fraud or corruption convictions, NO tender default listing. - Mbd 9 (certificate of independent bid determination): certifies the price was set independently with NO collusion. - Proof of good standing with municipal accounts (latest municipal account, lease agreement, or affidavit). - Authority for signatory certificate (company, partnership, joint venture, sole proprietor, or close corporation). - Banking details declaration with bank letter. - Joint venture agreement if bidding as a joint venture. - Schedule of proposed sub-contractors. - Experience of tenderer and reference letters. - Record of addenda. - Specialisation schedule completed and signed. Submission requirements: - bids must be placed in the tender box on the ground floor of umgungundlovu district municipality, 242 langalibalele street, pietermaritzburg, before 12h00 on the closing date. - Envelope must be sealed and clearly marked with the tender number and description. - Bids must be properly bound. - Late tenders, including those sent by fax or email, will not be considered. - Bids must be submitted on the official forms provided, not re-typed. - Non-attendance at the compulsory briefing session renders the bid invalid. - Failure to submit proof of valid professional body membership results in disqualification. - Bidders must be registered on the central supplier database (csd). - Bids must remain valid for 90 days from the closing date.
