RFQ: ad-hoc employee transport services for iziko museums staff (12-Month contract)

Scope: Provide ad-hoc employee transport services for Iziko Museums staff across various locations (including high-risk areas) for 12 months. Services: (1) Must hold a Charter Service or Staff Service Operating License. (2) Drivers and vehicles must have valid certifications, including PrDP and passenger liability insurance. (3) Door-to-door transport on agreed times, with flexibility for early/late schedules. (4) Vehicles must be reliable, legally compliant, and designed for passenger transport (no after-market conversions). (5) Maintenance costs are the supplier’s responsibility. (6) Additional vehicles must be supplied when necessary. (7) Signed trip registers required after each delivery. (8) Staff must be treated respectfully. Vehicle Categories: A (Sedan, 4 passengers), B (7-Seater), C (15-Seater). Service Areas: Mitchells Plain, Lavender Hill, Retreat, Grassy Park, Bonteheuwel, Gugulethu, Delft, Bellville, Parow, Dunoon, Khayelitsha, Nyanga, Langa, Somerset West, Atlantis, Darling, Eerste River, Elsies River, Clarke Estate, Wallacedene, Bishop Lavis, N2 Gateway, Paarl. Operational Requirements: Filter collection lists, group employees by routes, ensure vehicle inspections, and report challenges in writing.

Pricing must be submitted as a rate per fully seated vehicle (VAT excluded) for trips from Iziko to destination or vice versa. Include a formal detailed quotation with a full cost breakdown (VAT, disbursements, escalations). Payment terms: (1) Based on deliverables for services rendered. (2) Only for work completed satisfactorily and invoices approved by the project manager. Offers must remain valid for 60 days from the bid closing date. No price variations allowed after award.

Mandatory documents: (1) Central Supplier Database (CSD) Report with supplier number and company details (www.csd.gov.za). (2) Valid B-BBEE Certificate or Sworn Affidavit. (3) Signed Request for Quotation (RFQ) document. (4) SBD 4 (Bidder’s Disclosure) and SBD 6.1 (Preference Points Claim Form). Disqualification risks: (1) Bidder or associates listed on the Register of Tender Defaulters or List of Restricted Suppliers. (2) Incomplete or incorrect bid documents. (3) Late submission. (4) Non-registration on CSD or non-compliant tax status. (5) Failure to submit required administrative documents. Additional: Bidder must declare any state employment, relationships with Iziko employees, or interests in related enterprises. Collusive bidding or fraudulent claims may result in disqualification, penalties, or criminal investigation.
