Must be registered on Central Supplier Database with valid supplier number. Must provide valid B-BBEE certificate or sworn affidavit. Must provide proof of certification to work on the specified machines. Must not be listed on Register for Tender Defaulters or List of Restricted Suppliers. Must complete and submit all mandatory SBD forms. Must comply with preferential procurement regulations (80/20 system).

• Scope: Servicing, inspection, load testing, and issuing compliance certificates for material handling equipment over a 3-year period.
• Equipment includes: Stacker - Pramac TX12/25, Personal Lift Upright UL25, Scissor Lift Genie GS-2646, Scissor Lift Genie GS-1930.
• Deliverables: Annual servicing for four machines, one service per machine per year, totaling 12 services over 3 years.
• First service of all four machines must be completed within 2 weeks after purchase order issuance.
• Include all travel to Iziko South African Museum and Iziko National Gallery in the call-out fee.

• Specific goals points are awarded for B-BBEE status based on black ownership: 50% or more = 20 points, less than 50% = 10 points, 0% = 0 points.
• Proof required: B-BBEE certificate, company registration, identification documentation, and CSD report.
• Points claimed will be verified against available information, including CSD reports.

• The proposal and appended documentation may form the basis of a formal contract to be negotiated.
• A mere offer and acceptance does not constitute a formal contract until a written agreement/project charter is concluded.
• If specific goals are claimed fraudulently or contract conditions are not fulfilled, Iziko may: disqualify the bidder, recover costs, cancel the contract, restrict the bidder from public sector business for up to 10 years, or forward the matter for criminal prosecution.
• Bidders must declare any state employment, relationships with procuring institution staff, or interests in other related enterprises.

• The 80/20 preference point system applies, with 80 points for price and 20 points for specific goals.
• Price points calculated using the formula Ps = 80(1 - (Pt - Pmin)/Pmin).
• Specific goals points are for B-BBEE status based on black ownership.
• Failure to submit proof for specific goals means no points are claimed.
