Tenders must be submitted in a sealed envelope marked with the tender number, title, closing date, and the bidder's name and address, placed in the Tender Box at the CoGHSTA Head Office, Larry Moleko Louw Building, 9 Cecil Sussman Road, Kimberley, 8301. The envelope must contain the completed Form of Offer and Acceptance (C1.1) and all returnable documents, completed in non-erasable black ink. One original plus one electronic scanned copy on a flash drive must be provided. Telephonic, telegraphic, telex, facsimile or e-mailed offers will not be accepted. A two-envelope procedure will not be followed. Tenders must be signed by a person duly authorised; joint ventures must include a notarised joint venture agreement. The tender offer must remain valid for 90 days from the closing date. Tenders will not be opened immediately after closing; prices will not be read out due to a two-stage evaluation. Disqualification risks include: failure to attend both the compulsory clarification meeting and site visit, failure to submit correct declarations on all SBD forms, missing proof of CSD, CIDB, NHBRC or company registration, missing authority to sign with director's certified ID copy, previous failure to complete a CoGHSTA project in the last 5 years, and failure to comply with the scope of work.

Two-stage evaluation: first functionality, then price and preference. Functionality minimum threshold is 70 out of 100 points. Functionality criteria and weights: Experience of Tenderer 40%, Project Staff Experience 30%, Plant and Equipment 18%, Bank Rating 12%. Experience of Tenderer: points awarded for completed projects of similar scope (construction of buildings valued at R1.5 million or more including VAT) in the last 5 years — 5 projects = 40 points, 4 = 32, 3 = 24, 2 = 16, 1 = 8. Project Staff Experience: Construction Manager (10 points) — minimum B-Tech in Engineering/Built Environment, registered as Pr.CPM/Pr.CM/Candidate, 10+ years = 10, 7-9 = 6, 3-6 = 3, <3 = 0; Site Agent (10 points) — minimum N.Dip in relevant field, 7+ years = 10, 5-6 = 6, 3-4 = 3, <3 = 0; General Foreman (6 points) — building trade certificate, 7+ years = 6, 5-6 = 4, 3-4 = 2, <3 = 0; Safety Officer (4 points) — SACPCMP registered, 2+ years = 4, <2 = 0. The scope is the construction of 30 BNG (Breaking New Ground) houses in Garies, Kamiesberg Local Municipality, for the Namakwa District Municipality. The works include civil engineering construction as per the SANS Standardised Specifications and the General Conditions of Contract (GCC) for Construction Works (3rd Edition) 2015. The contract includes appointment of a Community Liaison Officer (CLO) at a fixed remuneration of R7,500 per month. The contractor must use local labour as far as possible and comply with EPWP reporting. A Health and Safety Plan must be approved before construction starts; a Health and Safety File must be maintained on site and handed over at final completion. The contractor must submit the Occupational Health and Safety Agreement and a letter of good standing from the Compensation Commissioner within 14 days after commencement. The project is not an EPWP project but local labour is required. Site information and geotechnical data are provided in Part C4. Engineering drawings are in Volume 2.

The project is VAT exempt; the employer will not pay VAT (VAT payable 0%). The tendered sum will not be subject to escalation. Pricing must be submitted on the Pricing Data forms (C2.1 and C2.2). The successful tenderer must provide security in one of three options: a fixed performance guarantee of 10% of contract sum, or an additional deduction of 10% from each interim certificate, or a fixed performance guarantee of 5% plus 5% retention from each interim certificate. The employer may request a banker's report within 3 days if needed. The employer may approach the tenderer's banker or guarantor to verify financial strength. The tender offer must remain valid for 90 days.

Mandatory registrations: CIDB grading 5 GB or higher, active and in good standing at tender close; NHBRC registration valid and not expired; CSD registration (proof of CSD registration summary report required). Tax: original valid Tax Clearance Certificate or certified copy with verification PIN. B-BBEE: B-BBEE Status Level Certificate or certified copy; Exempted Micro Enterprises must submit a certificate from an Accounting Officer, SANAS-accredited Verification Agency, or Registered Auditor. Other required certificates: Letter of Good Standing from Department of Labour, UIF Certificate of Compliance, Certificate of Incorporation (company), Founding Statement (CC), Partnership Agreement, or ID (sole proprietor), and proof of company registration. Authority to sign on behalf of bidder with director's certified ID copy. Joint ventures: signed JV agreement, joint CSD, and separate SBD forms for each partner. Returnable forms include SBD 4 (Declaration of Interest) and SBD 9 (Certificate of Independent Bid Determination) — correct declarations are mandatory. Proof of locality in Northern Cape (e.g. municipal account, lease agreement, title deed) for preference points. CVs of project staff with qualifications and certifications. Proof of plant and equipment ownership or hire agreements. Bank rating letter not older than 1 month. Contractors who previously failed to complete a CoGHSTA project in the last 5 years will be disqualified.
