Scope: Provide medical surveillance and wellness services for 36 months. Services include: (1) Medical Surveillance: Routine exams (pre-placement, annual, exit, transfer), carcinogen exposure tests (lead, asbestos, mercury), return-to-work, disability retirement/insurance, referrals. (2) Employee Assistance Programme: Counselling (telephone, in-person, online), trauma response, managerial consultations, substance abuse support, wellness days/campaigns. (3) Immunisation: Hepatitis A vaccination for ±50 employees. Extent of services: Vital signs, audiometric/spirometry tests, physical/vision/urine tests, chest X-rays, fitness certificates, referrals, annual reports, 25-year medical record retention. Target: ±120 staff (may vary). Services must be rendered within 50km of JMLM (Churchill) or on-site if beyond this radius, during working hours (Mon-Fri). Competencies: Minimum 3 years’ occupational health experience; registration with HPCSA, SASOM, SASOHN, or SANC; ability to conduct exams at mobile office/surgery.

Pricing: Firm, VAT-inclusive rates only (no lump sums). Quote per person for each service (e.g., full medical exam, audiogram, immunisation). Pricing schedule (MBD3.1) must be completed.
