Submit the following returnable documents with your bid:

• SBD 4 Declaration of Interest form, fully completed and signed.
• Proof of authority to undertake remunerative work outside public sector employment (if applicable).
• Full details of all directors, trustees, shareholders, or members, including their identity numbers, tax reference numbers, and employee/PERSAL numbers (if applicable).
• Confirmation of any interests in related companies (whether or not bidding for this contract). If yes, provide full particulars.

Disqualification risks:

• Failure to submit proof of authority for public sector employees undertaking outside work.
• False or incomplete declarations may result in bid rejection or legal action.
