The following documents must be attached: fully completed and signed SBD 4 form; company profile with track record and photos of similar work; completed compulsory price schedule; at least three signed reference letters or purchase orders from clients; CSD supplier report; CIPC registration documents; valid SARS tax pin. Non-compliance with any compulsory requirement leads to disqualification, except tax non-compliance which allows a 7-day grace period for the preferred bidder. Bids must remain valid for 60 days.

Two-stage evaluation: first, 100% conformance to specifications is required to qualify for preference point evaluation; second, the 80/20 preferential procurement system applies (80 points for price, 20 for specific goals). Specific goals: B-BBEE status (max 4 points), Free State based business (max 4), youth ownership (max 4), women ownership (max 4), people with disabilities ownership (max 4). B-BBEE points: Level 1 = 4, Level 2 = 3, Level 3 = 2, Level 4 and below = 1, non-compliant = 0. Ownership points: above 50% = 4, above 40% = 3, above 25% = 2, above 10% = 1. Proof of claim must be submitted; failure to submit proof results in no points for that criterion but does not disqualify the bid.

Shape: custom-shaped as per attached reference photograph/design. Colour: as per attached reference photograph/design. Finish: antique brass finish. Design: supplier to design and manufacture badges in accordance with approved design/sample. Delivery to National Museum, Bloemfontein.

Bidders must demonstrate experience in design, manufacture, supply and delivery of custom metal badges, medals, lapel pins, commemorative coins, challenge coins, name badges or similar products. Company profile must include background, mission, values, organisational structure, track record and photographs of previous similar work. At least three signed reference letters or purchase orders from clients confirming successful delivery of similar products are required.

Firm cost-inclusive quotation including delivery and VAT if applicable. Price schedule must show amounts excluding VAT, sub-total, delivery costs, VAT at 15%, and total.
