Required returnable documents: 1) Completed and signed pricing schedule, 2) Completed and signed MBD forms (MBD 1, 3.3, 4, 6.1, 8, 9), 3) Declaration on State of Municipal Accounts, 4) Current CSD report (not older than 3 months), 5) Municipal accounts for bidder and ALL directors (not in arrears >90 days, not older than 3 months) or valid lease agreement/sworn affidavit, 6) Valid copies of all mandatory documents. Disqualification risks: Failure to quote for ALL items, failure to attach drivers’ valid PDP (for vehicles carrying 12+ passengers), failure to attach valid Road Transportation Permit (Public Operating License) for each vehicle. Quotations must be completed in black ink; no correction fluid allowed. Quote reference number in email subject line if submitting digitally.

Label envelope with RFQ number (JTC0032/2025-26), description, closing date (22 May 2026), and time (10H00). No emailed bids accepted. Late submissions disqualified. Required documents to attach: - Quotation on company letterhead. - Fully completed RFQ and pricing schedule (all parts quoted; omissions disqualify). - Valid certified B-BBEE certificate or sworn affidavit. - Valid Tax Clearance Certificate (SARS). - Recent full CSD report with CSD registration number (handwritten on RFQ front page). - Fully completed MBD forms (3.3, 4, 6.1, 8, 9). - Declaration on State of Municipal Accounts. - Proof of registration with SATSA, ATTA, or IATA (advantageous). - Municipal account proof (not in arrears >90 days) for company and all directors, or lease agreement, or affidavit if neither leasing nor owning property. - Letter confirming any price alterations (crossed out in ink, initialed; no correction fluid allowed). Disqualification risks: - Incomplete or unsigned bid forms. - Missing pricing schedule or parts. - Use of correction fluid on pricing. - False declarations on MBD forms.
