NO telegraphic, telephonic, telex, facsimile, email or late proposals accepted. - Bid box location: ground floor, treasury house, 145 chief albert luthuli street (formerly commercial road), pietermaritzburg, 3200. - Attention: financial management – supply chain management unit, KZN provincial treasury. - Each bid must be in a separate sealed envelope, marked with the bidder's name, address, bid number (bid 1266/2026-F) and closing date. Envelopes containing documents for other bids may be rejected. - Bids must be on official forms provided; they may not be retyped or redrafted. Photocopies are allowed but must bear original signatures. - Bidders must initial each page of the bid document. Alterations must be initialled; correcting fluid is prohibited. Do not include samples with bid documents. - Returnable forms (all must be completed, signed and submitted before closing): - sbd 1 part a (invitation to bid): bidder details and offer cover page. - Sbd 1 part b (terms and conditions) plus a resolution letter or authority to sign the bid if there is NO resolution. - Section c: declaration that csd information is correct and up to date. - Sbd 4 (bidder's disclosure): declares any employment by the state, relationships with the procuring institution, and interests in other enterprises. - Failure to comply with phase 1 administrative requirements results in the bid being rejected as non-responsive. - Bids received after the closing time are not considered. Proof of posting is not accepted as proof of delivery.
