- Submit your quotation via email to [email protected] By 11:00 AM on 13 july 2026. - The delivery address for physical submissions (if required) is ithemba labs, att: SCM department, old faure road, faure, 7131. - Late submissions will not be accepted. - You must complete and submit the following mandatory returnable documents: - sbd 4 – bidder’s disclosure, signed and completed. - Sbd 6.1 – Preference points claim form, signed and completed, with a b-bbee certificate or sworn affidavit (for local bidders). - A completed pricing schedule or formal quotation. - Proof of registration as a radiation protection dosimetry service provider on the south african national dose register. - A minimum of three (3) current, contactable written reference letters or project completion certificates for similar projects completed within the last 36 to 60 months. - Provide your csd summary report (www.csd.gov.za). - Changes made to the RFQ template or its terms after the closing date will not be considered. - Failure to provide mandatory requirements may lead to disqualification.
