Use official bid forms; do not re-type or alter formats. All returnable documents must be submitted: Mandatory (disqualification if missing), Scoring (zero score if missing), or Essential (opportunity to resubmit). Sign, stamp, and date every page. Late bids are disqualified. Alterations to bid prices must be initialled by the signatory. Bids must be in ZAR unless specified otherwise. For joint ventures, submit a signed JV agreement or written intent with percentage splits. Foreign bidders must provide a Power of Attorney if applicable. Ensure all mandatory documents (e.g., HPCSA certificates, dispensing licences, hazardous biological waste disposal permits, subcontracting agreements) are included. Bids must be sealed and marked with the bid number and subject.

Evaluation consists of multiple stages: (1) Administrative responsiveness: Timeliness, completeness of returnable documents, valid signatures. (2) Substantive responsiveness: Compliance with legal/qualification criteria, priced offer. (3) Technical/Functionality: Minimum 80% score required. Weighted criteria: Competency and equipment (20%), qualified occupational health nurses (SANC/HPCSA/SASOHN) (10%), enrolled nursing assistants (SANC/SASOHN) (10%), technician qualifications for audiometry/spirometry (SASOHN) (10%), clinic manager experience (10%), Occupational Health Medical Practitioner (OHMP) experience (15%), radiation competency certification (10%), biological waste handling procedures (7.5%), decontamination procedures for equipment/facilities (7.5%). (4) Price: 80 points, weighted scoring formula (only unconditional discounts counted). (5) B-BBEE: 20 points via preference points claim form. Final score combines technical (80%) and B-BBEE (20%). Disqualification triggers: Missing mandatory documents, non-responsiveness, or scoring below 80% in technical evaluation. Parallel evaluation may occur; passing one stage does not guarantee progression.

Scope: Provision of clinic management services for 36 months, specializing in occupational health services for high-risk industries (petrochemical, chemical, mining, manufacturing). Mandatory technical requirements: (1) Registered practice with HPCSA (valid certificate), (2) Registered nurse with dispensing licence, (3) Occupational Medical Practitioner with MBCHB/Diploma in Occupational Health, valid HPCSA/SASOM registration, (4) Hazardous biological waste disposal permit, (5) Signed subcontracting agreements and permits if applicable. Additional technical criteria: Competency in core business with 5+ years experience in specified industries (20%), qualified occupational health nurses (SANC/HPCSA/SASOHN) (10%), enrolled nursing assistants (SANC/SASOHN) (10%), technician qualifications for audiometry/spirometry (SASOHN) (10%), clinic manager experience (10%), OHMP experience (15%), radiation competency certification (10%), written procedures for handling and storage of biological waste (7.5%), formal decontamination procedures for equipment/facilities (7.5%).

Pricing must be in ZAR, firm for the contract duration, and submitted in the official Price Schedule (Annexe A). Payment terms: Within 30 days of receipt of a valid tax invoice. VAT exclusive; VAT must be shown separately at the applicable rate. Expenses (e.g., travel, accommodation) require prior written approval from Foskor. Foreign currency payments require South African Reserve Bank (SARB) approval. Performance bonds or guarantees may be required post-award. Prices subject to confirmation are disqualified. Conditional discounts are calculated from the date of receipt of correct month-end statements. Foskor reserves the right to review and adjust fees for new or additional work. Reimbursement for approved expenses is at Foskor’s discretion.

Mandatory compliance: (1) CSD registration (all bidders, including JV partners), (2) Tax Compliance Status (TCS) PIN or valid tax clearance certificate (SARS verification), (3) Valid B-BBEE certificate (consolidated for JVs), (4) Compliance with Employment Equity Act (Section 53), (5) HPCSA/SANC/SASOHN registrations for medical personnel, (6) Hazardous biological waste disposal permits, (7) Proof of authority for bid signatories (e.g., company resolution or Power of Attorney for foreign bidders). Disqualification triggers: Missing mandatory documents, non-compliance with tax/CSD, submission of incorrect information, or blacklisting on National Treasury’s Database of Restricted Suppliers. Blacklisting applies for fraud, corruption, breach of contract, or litigation in bad faith. Foreign bidders must comply with local content and tax obligations if applicable. Subcontracting to blacklisted entities is prohibited.
