Duly completed and signed SDB 4 (Bidder’s Declaration), 5. Duly completed and signed SDB 6.1 (Preference Points Claim Form). Quotations must be on company letterhead or official template, include company details, contact information, VAT number (if applicable), and authorized signature. Prices must be inclusive of VAT and remain valid for 60 days after the closing date. No queries allowed 12 hours prior to closing.

Disqualification risks: 1. Listing on Register for Tender Defaulters or List of Restricted Suppliers, 2. Submission of incomplete or late returnable documents, 3. False declarations in SDB 4 or SDB 6.1 forms. Evaluation: 80/20 preference point system applies (80 points for price, 20 for specific goals). Specific goals: Women (6 points), Youth (6 points), People with Disabilities (4 points), Black People (4 points). Points claimed must be substantiated with proof (e.g., CIPC documents, certified IDs, disability confirmation letters). Lowest acceptable tender determines price scoring (Ps = 80(1 - (Pt - Pmin)/Pmin)).
