• Use official forms only (SBD 1, SBD 3.1, etc.). Do not re-type or alter forms; photocopies are allowed but must bear original signatures.

• Submit a physical sample of the 15kg storage box (equivalent to the document warehouse standard) to the new help desk by 11h00 on the closing date. Failure to submit a sample will disqualify the bidder.
• Do not include bids in packages containing samples.
• Initial all pages of the bid document. Use of correcting fluid is prohibited; all alterations must be initialed.

Evaluation uses the 80/20 preference point system:

• Price: Maximum 80 points. Formula: Ps = 80(1 - (Pt - Pmin)/Pmin), where Pt = tender price, Pmin = lowest acceptable tender price.
• Specific Goals: Maximum 20 points for Exempted Micro-Enterprise (EME) or Qualifying Small Enterprise (QSE) status. Documentary proof required: EME/QSE Sworn Affidavit (signed and dated by Commissioner of Oaths) or SANAS Certificate.
• Total points: 100 (80 for price + 20 for specific goals).
• Bidders must indicate how they claim points for each preference point system. Failure to submit required proof results in no points for specific goals.

Mandatory Requirements (disqualification if not met):

• Registration on the Central Suppliers Database (CSD).
• Submission of a physical sample of the 15kg storage box by the closing date.

• Delivery period: 1 week upon receiving a purchase order.
• Delivery basis: All delivery costs must be included in the bid price for delivery to Mayville Regional Office.

• Bidders must declare that the information on the CSD is correct and up to date. Incorrect or outdated information may lead to disqualification.

• A B-BBEE Status Level Verification Certificate or Sworn Affidavit (for EMEs and QSEs) must be submitted to qualify for preference points.

Disqualification:

• Bids will not be considered from persons in the service of the state, companies with directors who are persons in the service of the state, or close corporations with members who are persons in the service of the state.
• Bidders listed in the Register for Tender Defaulters or the List of Restricted Suppliers will be automatically disqualified.

Bidder’s Disclosure:

• Complete the SBD 4 form to declare any conflicts of interest, relationships with state employees, or interests in related enterprises.
• Collusive practices are prohibited and may result in criminal prosecution or restriction from conducting business with the public sector for up to 10 years.

Contract terms include:

• The successful bidder must sign a written contract form (SBD 7).
• The offer must remain valid for 90 days from the closing date of the bid.
• Payment will be made within 30 days after receipt of an invoice accompanied by a delivery note.
• All delivery costs must be included in the bid price for delivery at the prescribed destination.
• The bid price must include VAT and all applicable taxes (VAT, PAYE, income tax, UIF, skills development levies).
• The contract is subject to the Preferential Procurement Policy Framework Act, 2000, and the Preferential Procurement Regulations, 2022, as well as the General Conditions of Contract (GCC) and any special conditions of contract.
• The Department reserves the right not to award the lowest bidder.
