This document is an OHS (Occupational Health and Safety) evaluation template for high-risk tenders. It does not contain standard submission guidelines. Bidders must submit the following OHS returnable documents: • Annexure B - Signed acknowledgement of Eskom's OHS legal and other requirements form • Health and Safety Plan aligned with project scope and risks • Detailed costing for Health and Safety management (broken down, not lump sum) • Baseline OHS Risk Assessment (BRA) with methodology • Valid Letter of Good Standing (COIDA or equivalent) • OHS policy signed by CEO (compliant with OHS Act Section 7) • OHS Competency documentation: CVs, qualifications and certificates Note: For explanatory notes on OHS requirements, refer to document 240-77433139 Annexure A: Supplier Risk Category

• Obtain tender documents (including supporting forms) from Office 104, First Floor, Revenue Section, Ground Floor, Municipal Building, Dan Tloome Complex (Monday‑Thursday 08:00‑16:30, Friday 08:00‑13:30). A non‑refundable fee of R2 000 is payable at the cashier. • Complete the tender in strict accordance with the conditions and tender rules contained in the tender document. • Place the completed tender form and all supporting documents (including Tax Clearance Certificate and certified copies of directors/owners ID) in a sealed envelope clearly marked “TENDER 13/2026 – COMPLETION OF THE DISASTER MANAGEMENT CENTRE AT JB MARKS LOCAL MUNICIPALITY”. • Deliver the sealed envelope to the tender box of the JB Marks Local Municipality, Supply Chain Management Unit, Wolmarans Street, Potchefstroom – no facsimile, e‑mail or post submissions are accepted. • Ensure VAT is included in all quoted prices. • Register on the Central Supplier Database (CSD) (https://secure.csd.gov.za) before submission; non‑registration will lead to disqualification. • Late submissions (after 12:00 pm on 22/06/2026) will be rejected.
