Required returnable documents: SBD 1 (Invitation to Bid), SBD 4 (Bidder’s Disclosure Form), SBD 6.1 (Preference Claim Form), SBD 3 (Pricing Schedule), proof of CSD registration, and tax verification PIN (SARS or CSD). Evaluation phases: 1) SCM compliance (returnable documents), 2) Mandatory/technical requirements, 3) Technical/functional evaluation (70% minimum to progress), 4) Price and specific goals (80/20 system). Mandatory technical requirements: Equipment calibration certificates, Professional Indemnity Insurance, Public Liability Insurance. Technical/functional scoring (100 points total): Company experience (10), reference letters (10), team member experience in structural assessment (15), qualifications in civil/structural engineering (15), GPR/cover meter experience (10), GPR/cover meter qualifications (10), ECSA professional registration (30). Price scoring: 80 points for lowest price (Ps = 80(1 - (Pt-Pmin)/Pmin)). Specific goals: 20 points (B-BBEE, black women ownership, youth ownership). Preference points require valid B-BBEE certificate or sworn affidavit.

Scope: Specialist structural assessment of suspended slab investigation. Deliverables: Site investigation report (PDF), level survey results, reinforcement scan results, marked-up drawings (PDF and editable), concrete test results, photographic record, summary of observations. Work includes: Slab deflection/level survey (identify sagging, rotation, differential levels, uneven profiles; reference to datum; mark areas of concern), reinforcement scanning (suspended ground floor slab; beam/slab interfaces; crack zones; record positions, sizes, spacing, cover, congestion; mark positions on site; provide scan outputs), Schmidt hammer testing (concrete surface hardness; correlate to strength ranges). Bidder must: Visit site prior to quotation if required, confirm access/limitations, provide all equipment and competent personnel, comply with OHS regulations, indicate lead time, site duration, and reporting turnaround time.

Pricing: Quote in ZAR, inclusive of VAT and all costs (delivery, taxes, discounts). Complete SBD 3 pricing schedule in full. Validity period: 30 days from RFQ closure. Payment terms: Within 30 days from receipt of invoice after deliverables completion (Treasury Regulations 15.10.1 and 8.2.3). Price schedule includes: Mobilisation, site establishment, site visits, slab deflection survey, level survey drawings, reinforcement scanning, marking positions, scan data processing, Schmidt hammer testing, concrete test results, photographic record, specialist report, PDF/editable drawings, return visits, contingencies. Total bid price must be VAT-inclusive. For imported goods/services: Specify exchange rate, currency, and portion subject to fluctuations. Price negotiations may occur if prices are not market-related. SBD 3 price takes precedence over any conflicting quotes.

Mandatory: CSD registration (proof required), tax compliance (SARS TCS PIN or CSD number). Returnable documents: SBD 1 (fully completed/signed), SBD 4 (fully completed/signed), SBD 6.1 (fully completed/signed), SBD 3 (pricing schedule), CSD proof, tax verification. B-BBEE: Valid certificate or sworn affidavit required for preference points (SBD 6.1). Disqualification risks: Non-compliance with tax/CSD, incomplete/incorrect documents, fraudulent information, failure to meet mandatory technical requirements, misrepresentation, state employee conflicts, or listing on tender defaulters/restricted suppliers register. Consortia/JVs: Each party must submit separate TCS/CSD proof. Foreign suppliers: Must confirm RSA residency/branch/permanent establishment or income source via questionnaire.
