Bidders must submit three (3) reference letters for similar projects of similar value, along with the quotation document (Phase 2: Mandatory). Phase 2 (Mandatory): Service provider submits three (3) reference letters for similar projects of similar value, along with the quotation document.

SBD 3.2 contract price adjustment schedule with fields for applicable taxes, total bid price, and estimated time for completion.

Bidders must indicate a ceiling price based on the total estimated time for completion of all phases, including all expenses inclusive of all applicable taxes. Pricing schedule (SBD 3) must be completed with item description, bid price in RSA currency with all applicable taxes included. Rates quoted must be firm for the full period of contract (or provide details of basis for adjustments, e.g., consumer price index). Travel expenses only actual costs recoverable, proof must accompany certified invoices.

Bidders must complete and submit the following forms: SBD 1 (Invitation to Bid), SBD 3 (Pricing Schedule), SBD 3.2 (Price Breakdown for Rate Adjustment), SBD 4 (Bidder's Disclosure), SBD 6.1 (Preference Points Claim Form), SBD 7 (Contract Form), SBD 7.1 (Contract Form for purchase of goods), SBD 8 (Declaration of Bidder's Past Supply Chain Management Practices), SBD 9 (Certificate of Independent Bid Determination), and Authority to Sign. Bidders must be registered on the Central Suppliers Database (CSD). Bidders must declare any relationship with state employees or persons employed by the procuring institution. Bidders must not be listed on the Register for Tender Defaulters or List of Restricted Suppliers. Bidders must complete the Sworn Affidavit for B-BBEE Exempted Micro Enterprise (EME) or Qualifying Small Enterprise (QSE) to claim preference points.

The KZN Department of Public Works & Infrastructure (DPWI) warns prospective suppliers/bidders about a scam requesting suppliers to submit quotations or charge a non-refundable fee for quotation or tender documents. The KZN DPWI does charge tender deposit fees for adverts legitimately published in print media. Prospective suppliers/bidders are cautioned to contact the KZN DPWI when they suspect any scam or when fictitious purchase orders are placed using KZN DPWI's name. The bidder certifies that the information furnished in paragraphs 1, 2 and 3 is correct and accepts that the State may reject the bid or act against them in terms of paragraph 6 of PFMA SCM Instruction /22 on Preventing and Combating Abuse in the Supply Chain.

Delivery: Firm/not firm (delete if not applicable). 'All applicable taxes' includes value-added tax, pay as you earn, income tax, unemployment insurance fund contributions and skills development levies. Travel expenses: only actual costs are recoverable; proof of expenses must accompany certified invoices. The bidder must complete company name, registration number, and type of company/firm (Partnership/Joint Venture/Consortium, One-person business/sole propriety, Close corporation, Public Company, Personal Liability Company, (Pty) Limited, Non-Profit Company, State Owned Company). The signatory certifies that the information furnished is true and correct; preference points claimed are in accordance with the General Conditions; the contractor may be required to furnish documentary proof; if specific goals are claimed fraudulently, the organ of state may disqualify the person, recover costs, cancel the contract, recommend restriction from doing business for up to 10 years, and forward for criminal prosecution. EMEs and QSEs must complete the applicable sworn affidavit form to claim preference points.

4. Points awarded for specific goals

4.1. In terms of Regulation 4(2); 5(2); 6(2) and 7(2) of the Preferential Procurement Regulations, preference points

must be awarded for specific goals stated in the tender. For the purposes of this tender the tenderer will be

allocated points based on the goals stated in table 1 below as may be supported by proof/ documentation stated

in the conditions of this tender:

4.2. In cases where organs of state intend to use Regulation 3(2) of the Regulations, which states that, if it is unclear

whether the 80/20 preference point system applies, an organ of state must, in the tender documents, stipulate in

the case of—

(a) an invitation for tender for income-generating contracts, that either the 80/20 preference point system

will apply and that the highest acceptable tender will be used to determine the applicable preference

point system; or

(b) any other invitation for tender, that either the 80/20 preference point system will apply and that the

lowest acceptable tender will be used to determine the applicable preference point system,

then the organ of state must indicate the points allocated for specific goals for d 80/20 preference point system.
