Provision of packaging materials (single corrugated boxes, cold chain boxes, ice packs, solid heat-treated pharmaceutical pallets, continuous paper, tally ribbons, toners, drums, labels, stickers and printed tapes) for the eastern cape pharmaceutical depots for a period of 36 months.

Returnable documents that must be submitted with the bid:

• Completed SBD 1 – Summary Form of Offer (ink, signed)
• Completed SBD 3.2 – Pricing Schedules (ink, signed)
• Completed SBD 4 – Declaration of Interests (signed)
• Completed SBD 6.1 – Preference Claim Form (for specific goals points)
• Valid CIPC registration certificate (or proof of ownership/shareholding if registration not required)
• Proof of registration on the Eastern Cape Treasury Central Supplier Database
• For consortia/joint ventures: Letter of Intent or Joint Venture Agreement signed by all partners and the Joint Venture Disclosure Form
• Annexure‑A to‑E compliance certificates (ISO 9001, ISO 2784:1974, ISO 29862/29864, ISPM‑15, validation testing evidence, etc.)
• Any additional returnable forms referenced in the tender document

Failure to submit any of the above or to complete all fields will result in disqualification.

Functional Evaluation (Stage 3) – 30 points total:

• Bidders must complete the functional evaluation criteria scorecard and compliance checklist.
• Minimum threshold: 21 points (70%).
• Score < 21 points = non‑responsive, bid disqualified.
• Bidders meeting the threshold proceed to price evaluation; functional points are not used in price scoring.
• Functional scoring matrix applied as per tender document.

Mandatory compliance items (Annexures A‑E):

• Annexure A – Single corrugated boxes: ISO 9001 certification or signed letter/agreement if not the manufacturer.
• Annexure B – Cold chain boxes: Validation test proof for 2‑8 °C for 24‑48 h or signed letter/agreement.
• Annexure C – Solid heat‑treated pallets: ISPM‑15 certification or signed letter/agreement.
• Annexure D – Continuous paper: ISO 2784:1974 certificate or signed letter/agreement.
• Annexure E – Labels, stickers, printed tapes: ISO 29862/29864 certificates or signed letter/agreement.
• Failure to attach any required certificate or letter results in disqualification.

Additional compliance:

• Completion of specific goals claim form (SBD 6.1) for preference points.
• Submission of consortia/joint venture documentation where applicable.
• Submission of company registration documents (CIPC) and Central Supplier Database registration.
• Completion and signing of all required SBD returnable forms.
