Appointment of a service provider to provide occupational health and wellness programme for a period of three (3) months

SAFCOL requires a service provider to deliver a comprehensive Occupational Health and Wellness Programme for its 1139 employees across multiple business units in Limpopo, Mpumalanga, KwaZulu-Natal, and Gauteng for a period of three months. The programme must be holistic and ensure compliance with legislation, effective health risk management, and uniform service delivery.

Evaluation will be done in four phases:

• Phase 1: Administrative Compliance – Check for complete RFQ, signed SBD forms, tax compliance proof, company registration, B-BBEE certificate, CSD registration, and directors' IDs.
• Phase 2: Mandatory Requirements – You must meet all of the following:

• Minimum 5 years' experience providing Occupational Health and Wellness services to organisations with 1,000+ employees. Provide signed reference letters on client letterhead with contract dates and contact details.
• Occupational Medical Practitioner with MBChB qualification, valid HPCSA registration, practice number, and at least 3 years' OMP experience. Provide CV and copies.
• Detailed project plan covering all 12 scope of work areas listed in the technical specifications.
• Proof of a mobile medical clinic van meeting specified requirements (floor-to-ceiling cabinetry, desk, sink, bed, equipment, storage, power, climate control). Provide NaTIS registration or valid lease agreement with NaTIS.

• Phase 3: Mobile Clinic Inspection – A site inspection will be held at 20 Paul Kruger Street, ABSA Square, Nelspruit. Date and time will be communicated.
• Phase 4: Price and Specific Goals – 80 points for price, 20 points for specific goals. The 80/20 preference point system applies. Points for specific goals are awarded for 51% and above Black Owned entities (20 points). Proof required: CIPC documents, B-BBEE certificate/sworn affidavit, South African ID.

You must provide a comprehensive Occupational Health and Wellness programme for SAFCOL's 1139 employees across multiple provinces for three months. Key deliverables include:

• Occupational Health Medical Surveillance: Pre-employment, periodic, transfer, and exit medicals. Includes audiometry, lung function, vision screening, health questionnaires, physical exams, blood pressure, glucose, BMI, urine tests.
• COVID-19 and Pandemic Awareness: Screening, referral, workplace preparedness.
• Attendance at Scheduled Safety Meetings at plantations, offices, and processing plants.
• First Aider’s Kit Monitoring: Regular inspections for compliance.
• Villages Hygiene Inspections: Identify health and hygiene risks.
• Health Education and Empowerment: One-on-one and group sessions on chronic disease prevention, aligned with national health calendar.
• Primary Health and Chronic Condition Care via Private Public Partnership (PPP): Basic health assessments, vital signs, referrals.
• Chronic Disease Management: Maintain a confidential register, monitor conditions, schedule follow-ups, refer to specialists.
• Occupational Disease and Injury Management: Assess and manage work-related illnesses/injuries (e.g., noise-induced hearing loss), refer for physiotherapy, occupational therapy, radiology, psychology.
• Incapacity Management Process Due to Ill-Health/Injury: Support line management with formal counselling, medical assessments, documentation for insurer.
• HIV/AIDS Management Programme: Awareness campaigns, voluntary counselling and testing (VCT), psychosocial support, using PPP.
• Employee Assistance Programme (EAP): Provide 24/7 confidential counselling via telephone and one-on-one for:

• Psychosocial support (stress, anxiety, depression, relationships, grief, trauma).
• Workplace support (burnout, conflict, organisational change).
• Substance abuse support.
• Financial and legal guidance referral.
• EAP must be run by a registered psychologist or social worker, with a dedicated SAFCOL contact line.

• Reporting: Submit monthly, quarterly, and annual OH&W activity reports. Attend monthly SAFCOL OH&W management meetings.
• Staffing: Provide at least 1 Occupational Health Doctor for two clinics and one mobile clinic, three Occupational Health Nurses (one per clinic), one clinic cleaner for two clinics, and one administrator/technician per clinic.

• Provide a three-month detailed OH&W design programme and implementation plan.
• Submit monthly, quarterly, and annual OH&W activity reports.
• Attend SAFCOL's monthly OH&W management meetings.
• Key role players include SAFCOL's General Manager: Employee Relations and Wellness, Wellness Consultant, and Intern: Wellness.
• Your team must include an Occupational Medical Practitioner and support staff as specified in technical specifications.

• Your company must have at least 5 years' experience providing OH&W services to organisations with 1,000+ employees. Prove this with signed reference letters.
• Your Occupational Medical Practitioner must have an MBChB, HPCSA registration, and at least 3 years' OMP experience. Provide CV and copies.

• Ensure staff have relevant qualifications, registrations, and commit to continuous professional development.
• Maintain proper equipment and calibration.
• Participate in FSC audits, health & safety audits, company internal audits, service provider audits, and address corrective action requests.

• You must complete the pricing schedule provided in the document, listing all activities, estimated number of employees, and costs for three months inclusive of VAT.
• Clearly show totals per month and for the three-month period.

• Provide a detailed pricing schedule for all activities listed in the technical specifications, covering a three-month period.
• Prices must be firm and inclusive of VAT.
• The pricing schedule in the document includes line items for Occupational Health Medical Surveillance, COVID-19 awareness, incapacity management, occupational disease management, chronic disease management, safety meeting attendance, first aid kit monitoring, village hygiene inspections, health education, primary health care (PPP, no charge indicated), HIV/AIDS programme (PPP, no charge indicated), and Employee Assistance Programme.
• You must show total cost per month and total for three months, inclusive of VAT.

• Tax Compliance: Your tax status will be verified via CSD or SARS e-Filing. If non-compliant, you have 7 working days to rectify. Continued non-compliance leads to disqualification.
• B-BBEE: Submit a B-BBEE Verification Certificate from a SANAS-accredited agency OR a sworn affidavit if you are an EME/QSE.
• Central Supplier Database (CSD): You must be registered on the National Treasury CSD. If not registered, the successful bidder must register within 7 working days of award.
• Company Registration: Provide proof of company registration (e.g., Pty, Trust, CC).
• Directors' IDs: Provide ID copies of company directors.
• Disqualification Risks: You will be disqualified for:

• Non-compliant tax status after the 7-day rectification period.
• Fraudulent or inaccurate submitted information (e.g., false B-BBEE credentials, experience).
• False declarations on SBD forms or misrepresentation of facts.
• Being listed on the National Treasury's database of restricted suppliers and defaulters.
• Failure to quote in line with the specification.

• Additional Declarations: Complete the SBD4 Bidder's Disclosure form regarding state employment, relationships with SAFCOL officials, and other interests.
• Supplier Code of Conduct: You must confirm you have read and will adhere to SAFCOL's Supplier Code of Conduct.

• Submit a valid B-BBEE certificate or sworn affidavit.
• Specific goals points are awarded for 51% and above Black Owned entities (20 points). Proof required: CIPC documents, B-BBEE certificate/sworn affidavit, South African ID.
• Other specific goals listed include points for Black Women Owned, People with Disabilities, Black Youth Owned, and locality (RDP goals).

• The programme must ensure compliance with occupational health and safety legislation.
• Key focus areas include medical surveillance for occupational hazards, management of work-related illnesses and injuries, and promoting a healthy workforce.

• Contract Duration: Three months.
• Validity: Your offer remains binding and open for acceptance by SAFCOL for 60 days from the RFQ closing date.
• Award Communication: Successful bidders will be notified via an official purchase order or promissory note. Do not commence any work before receiving this.
• SAFCOL's Rights: SAFCOL reserves the right to accept any offer in full or in part, and not to make an appointment.
• Joint Ventures: If bidding as a joint venture, provide a consolidated B-BBEE certificate.
• Confidentiality: SAFCOL will process your personal data only for evaluation and award. Similarly, you must process SAFCOL's information confidentially.
• General Conditions: This RFQ is subject to National Treasury's General Conditions of Contract (GCC) and any Special Conditions of Contract (SCC).

• No work may start before receipt of an official purchase order or promissory note.
• Prices must be firm and inclusive of VAT.
• SAFCOL is not bound to accept the lowest or any offer.
• The offer is binding for 60 days from closing.
• SAFCOL may not appoint any provider.
• Joint ventures must provide a consolidated B-BBEE certificate.
