Required documents: Signed SBD Forms (SBD 1, SBD 4, SBD 6.1), POPIA Consent Form, B-BBEE Certificate or Sworn Affidavit, Full CSD Report with SARS Tax Compliance Status, NLC Pricing Schedule, and Municipality Certificate of Acceptability for Food Premises (Eastern Cape). Do not deliver goods or services without an official Purchase Order. Bidders must bring their own cutlery, plates, tablecloths, cups, and serviettes. Hard copies of additional documents (e.g., COIDA, qualifications) may be requested within 3 working days of an NLC SCM request.

Special conditions: Suppliers with existing contracts at Frere Hospital (or within the past 6 months) are ineligible due to rotation policy. Quoted prices must remain fixed for the contract duration unless otherwise advised. Department reserves the right to accept or reject any quote. Delivery/service is not contingent on payment by the department. If a conflicting contract exists, the RFQ contract will be cancelled.
