Submission method: email only - submission email: [email protected] - Only submissions received at this address on or before the closing date and time will be considered. - Quotations must be on the bidder's company letterhead. - All pages of the quotation must be signed by the responsible person. - The quotation must include a cost breakdown and be inclusive of VAT for vat-registered bidders. - The official RFQ document must be completed and signed; forms must not be retyped. - Faxed and emailed submissions are accepted; the bidder bears the risk of ensuring complete receipt by the siu by the due time. Returnable documents: - completed and signed RFQ document, including sbd 4 and sbd 6.1. - Sbd 4 (declaration of interest): discloses whether any director, shareholder or connected person is employed by the state or has a relationship with the siu; a false declaration disqualifies the bid. - Sbd 6.1 (Preference points claim form): claims b-bbee preference points under the 80/20 system. - Sbd 7 (contract form): to be signed by the successful bidder. - ID copies of company directors. - Cipc company registration. - SARS tax compliance status pin. - B-bbee certificate or sworn affidavit (eme/qse) to claim preference points. - Csd registration number and csd tax compliance summary report. - Proof of authority to sign (e.g. Company resolution). Disqualification risks: - late or incomplete submissions. - Unsigned quotation pages. - Omission of sbd 4 or sbd 6.1. - Non-compliant csd tax status. - Alterations to the tender document other than filling in bidder details and prices. - Being listed on the national treasury database of restricted suppliers or defaulters. - Being under investigation for corrupt activities. - Failure to pass siu internal integrity unit screening or state security agency vetting.
