Bids must be delivered to the bid box at the Central Supply Chain Management Unit, Old Boys Model School (OBMS), 310 Jabu Ndlovu Street, Pietermaritzburg, 3201, by the closing time. Late bids will not be considered. Bids must be submitted on the official forms provided, not retyped, and each page must be initialed. Bids must be in a sealed envelope marked with the bid number, closing date, and bidder's name and address. No bids by post, telefax, telegraphic, or electronic means will be accepted. Bids must be complete and include all returnable documents. Returnable forms include: SBD1 (Invitation to Bid), SBD4 (Bidder's Disclosure), SBD5 (National Industrial Participation Programme), SBD6.1 (Preference Points Claim), Authority to Sign the Bid, and a declaration that CSD information is correct. Proof of authority (e.g., company resolution) must be submitted. Failure to comply may render the bid invalid.

The applicable preference point system is the 80/20 system. Points are allocated as follows: Price 80 points, Specific Goals 20 points. Specific goals: companies that are at least 100% owned by Black African people are given 20 points. The 80/20 system applies for requirements with a Rand value up to R50 million; the 90/10 system applies for requirements above R50 million. The lowest acceptable bid will be used to determine the accurate system once bids are received. Bidders must be registered on the Central Supplier Database (CSD) at the time of bid closing. Bidders must be tax compliant and provide a SARS Tax Compliance Status PIN or CSD number. Bidders must not be persons in the service of the state, nor companies with directors who are state employees. Bidders must complete and submit SBD4, SBD5, SBD6.1, and an Authority to Sign form. Bidders claiming preference points for specific goals must submit proof of ownership (e.g., B-BBEE certificate or sworn affidavit for EMEs/QSEs).

Appointment of a panel of service provider(s) to render aeromedical services throughout the province of KwaZulu-Natal for the Provincial Department of Health (DoH) on a case per case basis, for a period of three (03) years. The National Industrial Participation (NIP) Programme applies to all government procurement contracts with imported content. The NIP obligation is benchmarked on imported content; contracts with imported content equal to or exceeding US$10 million will have a NIP obligation. The NIP obligation is 30% of the imported content. Successful bidders with contracts in excess of R10 million must submit details to the DTI within five working days after award. The NIP obligation agreement is between the DTI and the successful bidder.

The goods supplied shall conform to the standards mentioned in the bidding documents and specifications. ISO Standards means standards recognized by International Organization for Standardization. SABS means the South African Bureau of Standards. SANS means the South African National Standards.

Pricing schedules are included in Section J of the bid document. Prices charged by the supplier for goods delivered and services performed under the contract shall not vary from the prices quoted in the bid, with the exception of any price adjustments authorized in the Special Conditions of Contract or in the purchaser's request for bid validity extension. Performance security: within thirty (30) days of receipt of the notification of contract award, the successful bidder shall furnish the performance security of the amount specified in the SCC. The performance security may be a bank guarantee or irrevocable letter of credit.

Bidders must be registered on the Central Supplier Database (CSD) at the closing time of the bid, otherwise they will be disqualified. Bidders must ensure compliance with tax obligations and submit their unique Personal Identification Number (PIN) issued by SARS, or provide a CSD number if no TCS PIN is available. In bids involving consortia/joint ventures/sub-contractors, each party must submit a separate TCS certificate/PIN/CSD number. No bids will be considered from persons in the service of the state, companies with directors who are persons in the service of the state, or close corporations with members who are persons in the service of the state. Bidders must complete and submit SBD4 (Bidder's Disclosure), SBD5 (National Industrial Participation Programme), SBD6.1 (Preference Points Claim), and an Authority to Sign form. Bidders claiming preference points for specific goals must submit proof of ownership (e.g., B-BBEE certificate or sworn affidavit for EMEs/QSEs).

The contract shall be for a period of thirty-six (36) months. The General Conditions of Contract (GCC) form part of all bid documents and may not be amended. Special Conditions of Contract (SCC) supplement the GCC; in case of conflict, the SCC prevail. Key GCC clauses include: performance security, inspections and tests, packing, delivery, insurance, payment (within 30 days), contract amendments, assignment, subcontracts, delays, penalties, termination for default, force majeure, termination for insolvency, settlement of disputes, limitation of liability, governing language (English), applicable law (South African), notices, taxes and duties, NIP programme, and prohibition of restrictive practices.

This bid is subject to Treasury Regulations 16A issued in terms of the Public Finance Management Act, 1999, and the KwaZulu-Natal Supply Chain Management Policy Framework.
