The tenderer must submit a signed Form of Offer and Acceptance (C1.1), DPW-09 (PSB) particulars of projects, DPW-21 (PSB) record of addenda, DPW-16.1 (PSB) tender clarification meeting certificate (not applicable as no meeting is held), PA-11 bidder's disclosure, PA-16.1 (PSB) ownership particulars, PA-40 declaration of designated groups, and a resolution authorising the signatory (PA-15.1, PA-15.2, or PA-15.3). The professional team listed in the tender data is compulsory; failure to include all listed professionals will result in disqualification. Functionality evaluation criteria include: professional experience (10+ years = 5 points, 8-9 years = 4 points, 6-7 years = 3 points, etc.), key personnel qualifications, and past performance. The evaluation panel will assess the tenderer's capability based on submitted information and reference checks. An unacceptable risk on any single criterion disqualifies the tender.
