Quotations must be hand-delivered in a sealed envelope with the quotation number on the front to CENTLEC (Soc) Ltd, Supply Chain Management Offices, 30 Rhodes Avenue, Oranjesig, Bloemfontein, 9301. Emailed quotations will not be accepted. Documents are available on CENTLEC's website (www.centlec.co.za) and E-Tender (www.etender.gov.za).

Returnable documents — all must be completed, signed and submitted with the quotation:

• Quotation on business letterhead.
• Documents bound in book format (ring-bound), indexed and page numbered; loose or stapled documents are automatically disqualified.
• Copy of company registration (CK) documents.
• Valid original Tax Clearance Certificate or SARS PIN.
• CSD report and certified ID copies of company owner(s).
• Municipal services clearance certificate, or lease agreement with current bill and rates clearances, or current bill of account not owing more than 90 days (if landlord pays, signed lease and statement of account); for non-billed areas, an official council letter of residence (no affidavits accepted).
• MBD 4 (Declaration of Interest) completed in full and signed; MBD 9 completed in full and signed.
• Valid SANAS-approved B-BBEE certificate or Sworn Affidavit (failure to submit will not disqualify, but no preference points will be awarded).

Disqualification risks:
