Submit in two sealed envelopes: volume 1 (technical, compliance, specific goals) and volume 2 (financial: tender form c and price schedule). Deliver to PRASA tender drop off office, 3 old marine drive, cape town, 8000, before closing date and time. Include one original, copies, and an electronic version on a memory card or external hard drive. All pages consecutively numbered, signed by an authorised person, and certified copies where applicable. Bids must be submitted electronically via email to [email protected] In pdf format protected from modifications. The email subject line must include the bidder's name, tender NO and tender title. Bidders should email at least 60 minutes before closing to avoid delays; late submissions are rejected. All pages of the RFP document must be completed, signed and initialled. Returnable documents include: SARS pin, valid b-bbee certificate or sworn affidavit for EMEs/51% black-owned qses, TV licence statement or affidavit, certified company registration document, certified shareholders' certificates, certified ID copies of directors/members, last three years' audited/reviewed financial statements or management accounts, completed questionnaire (document c), sbd 4 declaration of interest, sbd 6.1 Preference points claim, sbd 8 declaration of past SCM practices, sbd 9 certificate of independent bid determination, acceptance of conditions of bid, and SABC vendor form. Registration on the central supplier database (csd) is required. Non-submission of mandatory documents results in automatic disqualification.
