Returnable documents: the bidder must submit the following documents with their tender: appointment letters for experience in ambient air quality data management and maintenance of the air quality monitoring stations; certified copies of qualifications for the project manager; cvs for the project manager and technician; and a letter of competence for skills transfer. The tender must be submitted on or before the closing time of 08:30 on the closing date.

Submit all required documents fully completed. Required forms: sbd1 (invitation to tender), sbd3.1 (Pricing schedule – firm prices), sbd4 (declaration of interest), sbd6.1 (Preference points claim form), sbd6.2 (Local production and content), sbd8 (declaration of bidder’s past SCM practices), sbd9 (certificate of independent bid determination), gcc (general conditions of contract), and terms of reference. Submit one (1) original, one (1) copy, and one (1) electronic copy (USB) of all bid documents. All forms must be completed in black ink; NO correction fluid allowed. Changes must be lined through and initialed. Bids must be sealed in an envelope labeled with bidder’s name, address, bid number (ART10/2025), and closing date. Postal bids must be registered mail; couriered bids must arrive by the deadline. Late bids will not be accepted. Incomplete or non-compliant submissions will be disqualified. Bids must include proof of csd registration (active, tax-compliant, non-restricted) with a supplier number starting with maaa. Include a valid original tax clearance certificate or tcs pin, and a valid b-bbee certificate (level 3 or better). Trusts, consortiums, or joint ventures must submit a consolidated b-bbee certificate per bid. Public entities and tertiary institutions must also submit b-bbee certificates. Company profile and proposal must align with the terms of reference. Technical threshold: 70%. Artscape reserves the right to award, not award, or partially award the bid.

Submit tender as an original only (NO hard copies required). Do not retype or redraft tender forms. Use the two-envelope system: not applicable. Submit to: zululand district municipality offices, lot B-400 gagane street, ulundi, 3838 or private bag x76, ulundi, 3838. Mark outer package with reference number zdm 008/2025-2026 and tender title. Closing time: 12h00 on tuesday, 03 february 2026. Late, faxed, or emailed submissions will be rejected. Proof of posting is not accepted as proof of delivery. Tender validity period: 120 days from closing date. Acknowledge and sign all addenda, attach to returnable documents. Failure to adhere to addenda results in disqualification. Return all returnable documents (e.g., Mbd forms, declarations, certificates, schedules) fully completed and signed. Non-responsive or incomplete submissions may be disqualified.

Returnable documents: certified proof of qualifications (by commissioner of oaths) and professional registrations must be submitted with the bid. Failure to submit these will result in disqualification.

Returnable documents:

• Submit the completed bid form (part a) with all required fields filled; missing information may lead to rejection. • Attach the following returnable documents: - project plan (to be submitted within 14 days of award) outlining all eap services. - Cvs and certified qualifications of key personnel (clinical psychologist, social worker, sign‑language interpreter). - Reference letters (minimum 6 for company experience, with grading as per evaluation table). - Proof of physical address and infrastructure ownership/lease within rustenburg. - Proof of registration with required professional bodies (eapasa, sacssp, sama, hpcsa). - Any required bonds or guarantees as stipulated in the bid form. • Submit all documents in the prescribed format (hard copy or electronic as indicated in the bid package) before 08:30 on the closing date. • Late submissions or incomplete returnables will be disqualified.

• Complete and sign form a (tender and contract quality requirements) as listed in the list of tender returnables. • Provide a valid iso 9001 (or applicable qms) certificate from an accredited body. • Submit internal and external audit reports, including any non‑conformities and corrective actions. • Provide a draft contract quality plan (cqp) specific to the tender scope, meeting iso 10005 requirements. • Where required, include a draft inspection & test plan (itp) or quality control plan (qcp) for similar previous work. • Supply documented information on control of externally provided processes, roles, responsibilities and authority (e.g., Org charts, job descriptions, procedures). • Include records of management review meetings (agenda, minutes, attendance, reports). • All returnable documents must be submitted as per the list of tender returnables (document 240‑12248652).

• Submit two electronic folders via the national treasury e‑tender portal (https://admin.etenders.gov.za): – Folder 1: technical and compliance documents only (NO pricing). – Folder 2: pricing schedule (sbd 3.1) Only. • Returnable documents include the signed sbd 1 (legal information), sbd 4 (administrative disclosures), sbd 6.1 (Preference points), sbd 3.1 (Pricing), reference letters, oem certification letters, training plan, and all manuals. • Closing time is 11:00 AM on 02 february 2026 (as per bid invitation). All submissions must be received by this time. • Clarification questions must be submitted in writing to the contact email; nrf will distribute answers to all bidders within 2 working days. The last date for clarification questions is 7 working days after the closing date. • Disqualification risks: missing any mandatory sbd form, including financial information in the technical folder, late submission, or failure to meet the minimum technical specifications.
