Requirements: Use official forms (no retyping); photocopies allowed but must have original signatures; each page must be initialed; no correcting fluid; alterations must be initialed. Envelope must display bidder’s name, address, bid number (ZNB 5622/2026-H), and closing date. Mandatory forms: Section B (CSD registration proof), Section C (CSD info declaration), Section E (Bidder’s Disclosure), Section G (Preference Points Claim), and initialed Annexure A (GCC). Scoring uses the 80/20 preference point system: 80 points for price, 20 for specific goals. Tie-breakers: Highest B-BBEE score wins if total points are equal; if functionality is evaluated and scores are tied, highest functionality score wins; if all else is equal, award decided by lot.

Scope: Process and collect claims for KZN Department of Health, including Injury-on-Duty supplier claims, Road Accident Fund (RAF) supplier claims (per Section 17(5) of RAF Act No. 56 of 1996), and departmental debts (per PFMA Chapter 5, Section 38(C)). Services: Comprehensive, all-inclusive processing of RAF/COID claims and debt collection (ex-employees, deceased debtors with open estates, supplier debtors). Deliverables: Must meet or exceed specifications; schedules for invoice values (12-month period per department) and departmental debts available on request. Contract duration: 36 months.
